PSA、FPSA和FPSA/TPSA联合血清检测在前列腺疾病鉴别诊断中的价值评价

摘　　要：目的探讨PSA、FPSA和FPSA/TPSA联合血清检测在前列腺疾病鉴别诊断过程中的价值.方法选取2018年5月~2020年5月来自本院男性健康体检者、前列腺癌患者、前列腺增生患者各60例作为研究对象,运用化学发光法对PSA、FPSA进行检测,并计算FPSA/TPSA比值.结果当TPSA=(0~4)μg/L时,BPH组、PCa组分别有25例、3例,显著低于健康对照组(P<0.05);当TPSA=(4~15)μg/L时,BPH组、PCa组分别有34例、30例,显著高于健康对照组(P<0.05),但两组间比较无差异(P>0.05);当TPSA值>15μg/L时,PCa组患者占比显著高于健康对照组、BPH组(P<0.05),健康对照组、BPH组对比,差异无统计学意义(P>0.05);三组PSA、TPSA和FPSA/TPSA对比,BPH组、PCa组PSA、TPSA指标水平显著高于健康对照组(P<0.05);PCa组F/T比值为(3.56±1.85)%,显著低于其他两组(P<0.05).结论PSA、FPSA和FPSA/TPSA联合血清检测用于对前列腺疾病的鉴别诊断,可弥补单种指标检查的不足,提升灰区中PCa检出率,提高特异度,从而提升前列腺疾病鉴别诊断的准确率,具有推广价值.0bjective To investigate the value of combined serum detection of PSA,F PSA and FPSA/TPSA in the differential diagnosis of prostate diseases.Methods 60 male healthy subjectsprostate cancer patients and prostatie hy perplasia patients from our hospital from May 2018 to May 2020 were selected as study subjects,and PSA and FPSA were deteeted by chemiluminescence method,and FPSA/TPSA ratio was calculated.Results When TPSA=(0~4)ug/L,there were 25 cases and 3 cases in BPH group and PCa group,respectively,which were significantly lower than that in healthy control group(P<0.05);When TPSA=(4~15)ug/L,there were 34 cases and 30 cases in BPH group and PCa group,which were significantly higher than that in healthy control group(P<0.05),but there was no difference between the two groups(P>0.05);When TPSA value was>15 ug/L,the proportion of PCa group was sigificantly higher than that of healthy control group and BPH group(P<0.05).There was no signifcant difference between healthy control group and BPH group(P>0.05).Compared with the three groups of PSA,TPSA and FPSATPSA,the levels of PSA and TPSA indicators in BPH group and PCa group were significantly higher than those in the healthy control group(P<0.05);the F/T ratio in PCa group was(3.56±1.85)%,which was significantly lower than that in the other two groups(P<0.05).Conc lusion The combined serum detection of PSA,FPSA and FPSA/TPSA for the differential diagnosis of prostate diseases can make up for the deficiency of single index examination,improve the detection rate of PCa in grey area,improve the specificity,and thus improve the accuracy of differential diagnosis of prostate diseases,which has the popularization value.
