摘　　要：目的:了解上海中医药大学附属龙华医院(以下简称"我院")门诊及住院病房首乌藤、何首乌和制何首乌的使用情况,促进合理用药,提高用药安全性。方法:利用我院HIS系统,抽取2019—2020年门诊、住院患者处方,共207398张。运用我院自行研发的处方分析系统PA软件(软件登记号:2017SR012498),对首乌藤、何首乌和制何首乌的使用人群、适应证、使用科室、用法用量以及不良反应发生率等进行专项点评,并对点评结果进行统计分析,判断用药的合理性。结果:首乌藤使用最多,其次为制何首乌,何首乌使用最少;在门诊,首乌藤的处方数为163327张,约为制何首乌处方数(36278张)的4.5倍,约为何首乌处方数(121张)的1349倍;在住院病房,首乌藤的处方数为6251张,约为制何首乌处方数(1421张)的4.4倍。首乌藤和制何首乌多用于肿瘤科,与两者的功效相符合;何首乌多用于专家门诊和风湿科,反应出临床使用何首乌比较谨慎,与其功效也相符合。首乌藤、何首乌和制何首乌均存在不同程度的超剂量使用现象。门诊和住院病房的用药情况基本一致。首乌藤、何首乌和制何首乌致肝损伤发生率均<1%,属于偶见不良反应。结论:在注意好辨证论治、掌握用量、注意配伍药物、不长时间使用、不滥用和不用于有肝病史患者的情况下,可以较安全地使用蓼科植物何首乌的根和藤。OBJECTIVE:To investigate the application status of Polygoni Multiflori Caulis,Polygoni Multiflori Radix and Polygoni Multiflori Radix Preparata in outpatient and inpatient department of Longhua Hospital Shanghai University of Traditional Chinese Medicine(hereinafter referred to as"our hospital")from 2019 to 2020,so as to promote the rational drug use and improve the drug safety.METHODS:A total of 207398 prescriptions for outpatients and inpatients from 2019 to 2020 were extracted by HIS system of our hospital.The prescription analysis system PA software(software registration number:2017 SR012498)developed by our hospital was used to perform prescription review on the users,indications,departments,usage and dosage and the incidence of adverse drug reactions of Polygoni Multiflori Caulis,Polygoni Multiflori Radix and Polygoni Multiflori Radix Preparata.Evaluation results were statistically analyzed to judge the rationality of medication.RESULTS:The use of Polygoni Multiflori Caulis was the most,followed by Polygoni Multiflori Radix Preparata and Polygoni Multiflori Radix.In the outpatient department,there were 163327 prescriptions of Polygoni Multiflori Caulis,which was about 4.5 times of the prescriptions of Polygoni Multiflori Radix Preparata(36278 prescriptions),and about 1349 times of the prescriptions of Polygoni Multiflori Radix(121 prescriptions).In the inpatient ward,there were 6251 prescriptions of Polygoni Multiflori Caulis,which was about 4.4 times of the prescriptions of Polygoni Multiflori Radix Preparata(1421 prescriptions).Polygoni Multiflori Caulis and Polygoni Multiflori Radix Preparata were mostly used in oncology department,which was consistent with the efficacy of both.Polygoni Multiflori Radix was mostly used in specialist clinics and rheumatology department,which reflected that the clinical use of Polygoni Multiflori Radix was more cautious and was consistent with its efficacy.Polygoni Multiflori Caulis,Polygoni Multiflori Radix and Polygoni Multiflori Radix Preparata all had different degrees
