气管插管麻醉和喉罩全麻在宫外孕腹腔镜手术中的应用效果对比

摘　　要：目的探讨气管插管麻醉和喉罩全麻在宫外孕腹腔镜手术中的麻醉效果。方法选取2019年1月至2020年4月江苏省淮安市淮阴医院收治的80例宫外孕患者为研究对象,采用随机数字表法分为对照组和观察组,各40例。对照组采用气管插管全身麻醉,观察组采用喉罩全麻,对患者的麻醉效果进行评估,比较两组不同时间点生命体征、术后3 d应激反应。结果观察组插管/喉罩前(T1)、插管/喉罩时(T2)、插管/喉罩后5 min(T3)时间点平均动脉压(MAP)、心率(HR)水平均低于对照组(P<0.05);观察组术后3 d皮质醇(COR)、内皮素(ET-1)及人类软骨糖蛋白(YKL-40)水平低于对照组(P<0.05);促甲状腺激素(TSH)水平高于对照组(P<0.05)。结论喉罩全麻用于宫外孕腹腔镜手术患者中对患者生命体征影响较小,能降低麻醉应激反应,值得推广应用。Objective To investigate the anesthetic effect of tracheal intubation anesthesia and laryngeal mask anesthesia in laparoscopic surgery for ectopic pregnancy.Methods A total of 80 patients with ectopic pregnancy admitted to Huaiyin Hospital of Huai'an City from January 2019 to April 2020 were selected as the research objects and divided into control group and observation group by random number table method,with 40 patients in each group.In the control group,tracheal intubation was used for general anesthesia,while in the observation group,laryngeal mask airway was used for general anesthesia.3 days after surgery postoperative anesthesia effect was evaluated,the vital signs and stress responses of the two groups were compared.Results The average arterial pressure(MAP)and heart rate(HR)levels of T1,T2 and T3 in the observation group were all lower than those in the control group(P<0.05).The levels of cortisol(COR),endothelin(ET-1)and human chondroglycoprotein(YKL-40)in the observation group were lower than those in the control group on 3 days after surgery(P<0.05),and the level of thyroid stimulating hormone(TSH)was higher than the control group(P<0.05).Conclusion Laryngeal mask airway general anesthesia in patients with ectopic pregnancy laparoscopic surgery has little impact on the vital signs of patients,can reduce the anesthetic stress reaction,is worthy of promotion and application.
