摘  要:目的:探讨新型冠状病毒肺炎(COVID-19)肺段感染的特征,及其与临床的相关性,为COVID-19的诊治提供一定参考。方法:回顾分析90例COVID-19患者的胸部CT影像及临床资料,对肺段感染的数目、位置、分布进行重点分析,统计肺段感染数目与临床分型的相关性。结果:胸部CT未见异常8例,异常82例,单个肺段感染12例,单侧肺感染24例,双肺感染58例。肺段病灶形态以斑片、球形为主,密度以磨玻璃样改变为主,实变相对较少。感染肺段在下背段、下外段、下后段中占比超过50%。不同临床疾病分型的感染肺段数不同,临床分型越重感染肺段数越多,差异均具有统计学意义(P <0.05)。结论:COVID-19病灶较多分布在双肺下背段、下外段、下后段。肺段感染个数与临床分型存在显著正相关,有利于临床对病程及严重程度进行评估。Objective:To explore the characteristics of lung segment infection in coronavirus disease 2019(CO⁃VID-19)and its clinical relevance,and to provide a certain reference for the diagnosis and treatment of COVID-19.Methods:A retrospective analysis was performed for the chest computed tomography(CT)findings and clinical da⁃ta of 90 COVID-19 patients,with special attention to the number,location,and distribution of lung segments infect⁃ed,and the correlation between the number of infected lung segments and clinical classification was calculated.Results:Chest CT showed no abnormalities in 8 cases,and abnormalities in 82 cases,among whom 12 had infection in a single lung segment,24 had infection in one lung,and 58 had infection in both lungs.The lung lesions were mainly patchy or spherical and appeared mostly as ground-glass opacities,with little consolidation.More than 50%of the in⁃fections occurred in the lower superior,lower lateral,and lower posterior segments.The number of infected lung seg⁃ments varied significantly across different clinical types of COVID-19;the more severe the clinical type,the greater the number of infected lung segments(P<0.05).Conclusion:COVID-19 lesions are mostly distributed in the low⁃er superior,lower lateral,and lower posterior segments of both lungs.There is a significant positive correlation be⁃tween the number of infected lung segments and the clinical classification of COVID-19,which is helpful for clinical evaluation of the course and severity of the disease.
