超声引导下压迫联合瘤腔注射凝血酶治疗医源性假性动脉瘤的临床观察

摘　　要：目的观察超声引导下压迫(UGG)联合瘤腔注射凝血酶(UGTI)治疗医源性假性动脉瘤(IPSA)的临床效果。方法回顾性在分析2018年1月至2020年1月于成都医学院第一附属医院行医源性动脉穿刺后并发假性动脉瘤患者的临床资料共48例,观察UGG联合UGTI治疗的效果。结果48例患者均成功封堵,注射凝血酶剂量300~500U,其中45例成功,3例二次注射成功,总治愈率为100%(48/48)。术后24h、1个月超声随访,假性动脉瘤瘤腔内均未见血流信号进入,且无破裂出血和远端动脉栓塞等并发症出现。结论超声引导下压迫联合凝血酶注射治疗IPSA,创伤小、费用低、操作简单、成功率高、复发率低,可作为IPSA治疗的首选方法,值得临床推广应用。Objective To observe the clinical effect of ultrasound-guided compression(UGG)combined with ultrasound guided thrombin injection(UGTI)in the treatment of iatrogenic pseudoaneurysm.Methods The clinical data of 48 patients with iatrogenic pseudoaneurysm after arterial puncture from January 2018 to January 2020 in the First Affiliated Hospital of Chengdu Medical College were retrospectively analyzed.And the therapeutic effect of UGC combined with UGTI was observed.Results All the 48 cases were successfully blocked with thrombin dose of300-500 U.Among them,45 cases were successful in the first injection,and 3 cases were successful in the second injection.The total cure rate was 100%(48/48).Ultrasound follow-up 24 hand 1 month after the operation showed no signal of blood flow in the pseudoaneurysm cavity,and no complications such as rupture,bleeding,and distal arterial embolism occurred.Conclusion UGC compression combined with GUTI in the treatment of iatrogenic pseudoaneurysm has the advantages of small trauma,low cost,simple operation,high success rate and low recurrence rate.It can be used as the preferred method for the treatment of iatrogenic pseudoaneurysm.And it is worthy of clinical application.
