分期论治、肝脾同调治疗多发性抽动症的临床经验

摘　　要：多发性抽动症(tourette’s syndrome, TS)是一种儿童常见的复杂性慢性神经精神障碍。近年来发病率较高,中医药治疗TS有较好效果。脾胃虚弱是TS发病基础,情绪变化是TS重要诱因,肝风内动是其证候核心,神魂不安是TS神志表现。在TS治疗过程中当标本兼治,急则治其标,缓则治其本,病情缓解之后还需注意收功之法。治疗以健运脾胃、平肝熄风、调和营卫、清心安神之法贯穿始终,兼以疏肝解郁、交通心肾之法。常用方剂有山五汤、桂枝加龙骨牡蛎汤等。结合临床病案对分期论治,肝脾同调儿童多发性抽动症的经验进行介绍。Tourette’s syndrome(TS) is a common complex neuropsychiatric disorder in children. In recent years, its incidence rate is high. It is found that traditional Chinese medicine has better effect on this disease. The spleen and stomach weakness is the basis of the disease, emotional changes are an important trigger of the disease, liver wind internal movement is the core of the syndrome of the disease, and restless spirit is the manifestation of the disease. In the process of treatment of this disease, the tips and roots are treated simultaneously. If the symptoms are urgent, the symptoms will be treated, and if the disease is relieved, the method of recovering meridians should be paid attention to. The methods of invigorating the spleen and stomach, regulating Ying and Wei, pacifying the liver, clearing the heart and tranquilizing mind run through the whole process, and the methods of soothing the liver and relieving depression, communicating the heart and kidney are also used. Commonly used prescriptions are Shanwu Decoction, Guizhi plus Longgu Oyster Decoction and so on. This paper introduces the experience of treating Tourette’s syndrome in children by stages in accordance with the clinical records.
