摘  要:目的探究新生儿转运系统在危重新生儿转运中的应用。方法回顾性分析2017年3月至2019年3月于本院应用新生儿转运系统进行转运的300例危重新生儿的临床资料。所有进行新生儿转运的危重儿家属均签署转运同意书,在转运途中予以患儿相应处理措施,包括清理呼吸道、气管插管、面罩给氧、鼻导管或头罩吸氧、静脉穿刺给药、镇静、抗惊厥、纠正内平衡、扩容、补碱、止血等,并对转运期间的用药以及相关操作记载有详备的转运记录。结果对区域内的各级医院间建立转运网以及转运疾病类型规定,其中早产儿30例、新生儿呼吸窘迫综合征30例、新生儿代谢酸中毒20例、新生儿呼吸道梗阻30例、新生儿溶血病12例、胎粪吸入综合征18例、惊厥25例、新生儿颅内出血25例、新生儿脑膜炎23例、新生儿缺血缺氧性脑病12例、新生儿休克25例、新生儿黄疸20例、严重窒息10例、重症肺炎20例。对本院不同年度危重新生儿转运结果进行分析,2017年3月至2018年3月未应用新生儿转运系统之前共转运120例,转运成功114例(95.00%),病死6例(5.00%);2018年4月至2019年3月应用新生儿系统后共转运180例,转运成功179例(99.44%),病死1例(0.56%);实行新生儿转运系统后的转运成功率明显高于应用转运系统前,比较差异有统计学意义(P<0.05)。结论在对危重新生儿进行转运时结合新生儿转运系统的相关操作要求进行转运,能显著提高对于危重新生儿的救治成功率,降低病死率,从而有效提高新生儿的出生质量。Objective To investigate the application of neonatal transport system in transport of critical neonates.Methods The clinical data of 300 critically ill newborns who were transferred by neonatal transport system in our hospital from March 2017 to March 2019 were retrospectively analyzed.For all critical infants who were in transit,the parents have signed the transfer consent.During the transportation,the children were taken corresponding measures to clean up the respiratory tract,endotracheal intubation,oxygen supply by mask,oxygen inhalation by nose tube or head cover,intravenous puncture,sedation,anticonvulsion,correction of internal balance,expansion,alkali supplement,hemostasis,etc.,and have detailed transportation records of the drugs used during the transportation and related operation records.Results There were 30 cases of premature infants,30 cases of neonatal respiratory distress syndrome,20 cases of neonatal metabolic acidosis,30 cases of neonatal respiratory obstruction,12 cases of neonatal hemolysis,18 cases of meconium aspiration syndrome,25 cases of convulsion,25 cases of neonatal intracranial hemorrhage,23 cases of neonatal meningitis and 23 cases of neonatal ischemic deficiency.There were 12 cases of oxygen encephalopathy,25 cases of neonatal shock,20 cases of neonatal jaundice,10 cases of severe asphyxia and 20 cases of severe pneumonia.The results of critical newborn transport in different years were analyzed.From March 2017 to March 2018,120 cases were transported before the application of neonatal transport system.114 cases(95.00%)were successfully transported and 6 cases(5.00%)died.From April 2018 to March 2019,180 cases were transported after the application of neonatal system.179 cases(99.44%)were successfully transported and 1 case died(0.56%).The success rate of transport after the implementation of neonatal transport system was significantly higher than that before the application of transport system,and the difference was statistically significant(P<0.05).Conclusion During the transporta
