集束化温柔分娩模式对高龄产妇分娩结局的影响

摘　　要：目的探讨集束化温柔分娩模式对高龄产妇分娩结局的影响。方法选取2017年12月—2018年10月本院产科进行产前检查并住院分娩的高龄产妇190例作为研究对象,按集束化温柔分娩模式执行时间分为对照组(2017年12月—2018年3月)和干预组(2018年4—10月)两组,对照组92例,干预组98例。对照组采用传统管理模式,干预组采用集束化温柔分娩模式。比较两组产妇在VAS评分、无指征剖宫产率、产程时间、产后2小时出血量、会阴情况、新生儿Apgar评分等指标上的差异。结果干预组高龄顺产产妇的VAS评分明显低于对照组,差异有统计学意义(P<0.05);干预组高龄产妇的无指征剖宫产率低于对照组,差异具有统计学意义(P<0.05);干预组高龄顺产产妇的会阴裂伤程度及切开率均低于对照组,差异具有统计学意义(P<0.05);两组高龄顺产产妇的产程时间、产后2小时出血量、新生儿Apgar评分比较,差异无统计学意义(P>0.05)。结论集束化温柔分娩模式能够减轻分娩疼痛、降低会阴损伤程度及切开率,且不会增加母儿不良结局的发生。Objective To explore the effect of cluster gentle delivery pattern on the outcome of childbirth in advanced maternal age.Methods A total of 190 cases of advanced maternal age those carried out prenatal examination and give birth in obstetrics of our hospital during December 2017 and October 2018 were enrolled as study subjects.They were divided into control group(December 2017 to March 2018)and intervention group(April to October 2018)according to the execution time of clustered gentle delivery mode,and there were 92 cases in control group and 98 cases in intervention group.Pregnant women in control group received traditional management mode and gentle delivery mode was implemented in intervention group.The VAS score,the rate of non-indicative caesarean section,delivery time,postpartum 2 hours hemorrhage,the vulva condition,neonatal Apgar score of the two groups were compared.Results The VAS score of the intervention group was lower than that of the control group,the difference was statistically significant(P<0.01).The the rate of non-indicative caesarean section of the intervention group was lower than that of the control group,the difference was statistically significant(P<0.01).The perineal laceration degree of the natural labour puerperant and perineotomy rate of the intervention group was lower than that of the control group,the difference was statistically significant(P<0.01).The differences of birth time,the amount of bleeding 2 hours after birth,and the neonatal Apgar score between the two groups were not statistically significant(P>0.05).Conclusions The clustered gentle delivery mode could reduce the pain of childbirth,reduce the degree of vulva damage and the rate perineotomy,and do not increase the occurrence of adverse outcomes in the mother and neonatus.
