摘 要:目的:探讨妊娠期糖尿病孕妇心率变异性变化。方法:选取阳春市妇幼保健院2018年1月至2020年4月收治的50例妊娠期糖尿病患者作为观察组,选取同期健康孕检孕妇50例作为对照组,年龄20~36岁,孕周24~40周。所有孕妇给予动态心电图监测,比较两组孕妇平均心率(AHR)、正常窦性RR间期的总体标准差(SDNN)、每5 min窦性RR间期均值标准差(SDANN)、24 h相邻连续性窦性RR间期差值的均方根(rMSSD)、相邻RR间期的差值超过50 ms百分比(PNN50)等时域指标,及总频率(TF)、低频谱功率(LF)、高频谱功率(HF)、LF/HF等频域指标。结果:观察组SDNN、SDANN、TF、LF、HF均低于对照组(P<0.05),两组PNN50、LF/HF、AHR、rMSSD比较,差异无统计学意义(P>0.05)。结论:妊娠期糖尿病孕妇自主神经调节功能下降,通过动态心电图能够发现心率变异性变化差异。Objective:To analyze the changes of heart rate variability in pregnant women with gestational diabetes mellitus.Methods:Fifty pregnant women with gestational diabetes mellitus in Yangchun Women and Child Health Hospital from January 2018 to April 2020 were enrolled in the study as the observation group,and 50 healthy pregnant women as the control group,with 20-36 years old,gestational age 24-40 weeks.Holter was performed in all pregnant women.The mean heart rate(AHR),standard deviation of NN intervals calculated over a 24-hour period(SDNN),standard deviation of the average NN intervals calculated over 5 minutes within a 24-hour period(SDANN),the root mean square of adjacent R-R interval difference within 24 hours(rMSSD),the percentage of adjacent NN R-R interval difference>50 ms(PNN50),total frequency(TF),low frequency spectral power(LF),high frequency spectral power(HF)and LF/HF were compared in the two groups.Results:The levels of SDNN,SDANN,TF,LF and HF were lower in the observation group than those in the control group(P<0.05),without statistical difference for PNN50,LF/HF,AHR and rMSSD(P>0.05).Conclusion:The autonomic nerve regulation function of pregnant women with gestational diabetes mellitus is decreased,and the difference of heart rate variability can be found by dynamic electrocardiogram.
