摘　　要：目的探讨血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、C-反应蛋白(CRP)、可溶性细胞间黏附分子-1(sICAM-1)、降钙素原(PCT)水平评估儿童重症肺的应用价值。方法120例社区获得性肺炎患儿纳入研究,根据病情的严重程度,分为重症肺炎组(n=52)和轻中度肺炎组(n=68)。另外选取同期进行健康体检的健康儿童60例作为对照组。比较肺炎组和对照组,重症肺炎组和轻中度肺炎组患者上述指标的差异,并分析其与临床肺部感染评分(CPIS评分)的相关性。结果肺炎组患者TNF-α、IL-6、CRP、sICAM-1及PCT水平均明显高于对照组(P<0.05)。重症肺炎组患者TNF-α、IL-6、CRP、sICAM-1、PCT水平及CPIS评分均明显高于轻中度肺炎组(P<0.05)。CPIS评分与TNF-α、IL-6、CRP、sICAM-1、PCT均呈正相关(r值分别为0.484,0.596,0.626,0.587,0.702,P<0.05)。结论TNF-α、IL-6、CRP、sICAM-1、PCT水平与肺部感染严重程度呈明显的正相关,对患儿肺炎病情判断、预后评估、用药指导等方面具有重要的临床意义。Objective To investigate the application value of the detection of serum levels of TNF-α,IL-6,CRP,sICAM-1 and PCT in evaluating the severity of children with severe pneumonia.Methods A total of 120 children with community-acquired pneumonia were enrolled in the study.According to the severity of the disease,who were divided into severe pneumonia group(n=52)and mild to moderate pneumonia group(n=68),at the same time,the other 60 healthy children who underwent physical examination in our hospital were enrolled as control group.The above indexes were observed and compared among the three groups,moreover,the correlation between the above indexes and CPIs score was analyzed.Results The levels of TNF-α,IL-6,CRP,sICAM-1 and PCT in pneumonia groups were significantly higher than those in control group(P<0.05),moreover,which in severe pneumonia group were significantly higher than those in mild to moderate pneumonia group(P<0.05).In addition the CPIS scores were positively correlated with TNF-α,IL-6,CRP,sICAM-1(P<0.05).Conclusion The levels of TNF-α,IL-6,CRP,sICAM-1 and PCT are positively correlated with the severity of pulmonary infection,which has important clinical significance in judging the condition of pneumonia,evaluating the prognosis and guiding the use of drugs.
