摘　　要：目的评估肿瘤坏死因子-α(tumor necrosis factor-alpha,TNF-α)、白介素-6(interleukin-6,IL-6)和microRNA-142的表达变化对2型糖尿病(type 2 diabetes mellitus,T2DM)足部感染患者早期诊断的价值。方法选取2017年3月至2019年3月治疗的T2DM并发早期足部感染患者60例为研究对象,设为研究组。同时纳入同期治疗的糖尿病患者,但无并发症的患者60例,设为对照组。比较2组患者空腹血糖(FPG)、糖化血红蛋白(HbA1c)、TNF-α和IL-6。检测比对2组患者microRNA-142和microRNA-144的表达水平。结果研究组FPG、HbA1c、TNF-α和IL-6血清含量均显著高于对照组(P<0.05)。研究组microRNA-142水平显著低于对照组(P<0.01),microRNA-144水平显著高于对照组(P<0.01)。研究组IL-6水平与microRNA-142呈负相关(r=-1.150,P<0.01)。结论TNF-α、IL-6和microRNA-142在T2D合并足部感染患者的诊断和病程监测上具有一定临床价值,并为糖尿病足的病理机制提供了新的方向,值得深入研究。Objective To investigate the significance of changes in expression of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and microRNA-142 in early diagnosis of food infection in patients with type 2 diabetes mellitus(T2DM).Methods Sixty patients with T2DM complicated by early foot infection who were treated in our hospital from March 2017 to March 2019 were enrolled as research group,at the same time,the other 60 diabetic patients without complications who were enrolled as control group.The fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),TNF-αand IL-6,and the expression levels of microRNA-142 and microRNA-144 were observed and compared between the two groups.Results The serum levels of FPG,HbA1c,TNF-αand IL-6 in research group were significantly higher than those in the control group(P<0.05).The levels of microRNA-142 in research group were significantly lower than those in control group(P<0.01),however,the levels of microRNA-144 were significantly higher than those in control group(P<0.01).The level of IL-6 in the research group was negatively correlated with microRNA-142(P<0.01).Conclusion TNF-α,IL-6 and microRNA-142 are of certain clinical value in the diagnosis and course monitoring of patients with T2DM complicated by foot infection,which provides an new direction for reseraching the pathological mechanism of diabetic foot.
