氢溴酸樟柳碱注射液治疗非动脉炎性前部缺血性视神经病变的临床观察

摘　　要：目的:评价氢溴酸樟柳碱注射液对非动脉炎性前部缺血性视神经病变的有效性和安全性。方法:111例病程不超过30d的非动脉炎性前部缺血性视神经病变患者随机分为两组。对照组37例采用常规治疗方法;试验组74例在对照组基础上加用氢溴酸樟柳碱注射液2 mg,ivd,qd。两组均连续治疗14 d。治疗前后分别对两组患者进行中心视力、30°静态视野、眼底镜检、视盘部视网膜光学相干断层扫描及视觉诱发电位等各项检查,比较两组疗效和药品不良反应发生率。结果:试验组总有效率和显效率均明显高于对照组(P<0.05)。治疗7 d后,两组患者视力较治疗前均有明显提升(P<0.05),组间差异无统计学意义(P>0.05);治疗14 d后,试验组视力、视野、视盘RNFL厚度、诱发电位检查P100波振幅及潜时等各项指标及其改善情况均优于对照组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论:氢溴酸樟柳碱注射液对非动脉炎性前部缺血性视神经病变有效、安全。Objective: To evaluate the efficacy and safety of anisodine hydrobromide injection in the treatment of non-arteritic anterior ischemic optic neuropathy. Methods: Totally 111 patients suffering from non-arteritic anterior ischemic optic neuropathy with a course of less than 30 days were randomly divided into two groups. The control group(n = 37) was treated with the conventional treatment method. The test group(n = 74) was treated with anisodine hydrobromide injection 2 mg,ivd,qd on the basis of the control group.Both groups were treated continuously for 14 days. The central visual acuity,30° static visual field,fundus microscopy,optic disc retina optical coherence tomography and visual evoked potential were compared between the two groups before and after treatment. At the same time,the incidence of adverse reactions was compared between the two groups. Results: The total effective rate and the obvious effective rate of the experimental group were significantly higher than those of the control group(P<0.05). After 7-day treatment,the visual acuity of the two groups was significantly improved when compared to that before treatment(P<0.05),and there was no significant difference between the groups(P>0.05). After 14-day treatment,the vision,visual field,optic disc RNFL thickness,evoked potential test P100 wave amplitude and latency as well as the other indicators in the test group were all better than those of the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion: Anisodine hydrobromide injection is effective and safe for non-arteritic anterior ischemic optic neuropathy.
