中老年手部慢性湿疹137例斑贴试验结果分析

摘　　要：目的:总结中老年手部慢性湿疹患者的常见致敏成分,为其防治提供依据。方法:回顾性分析2019年1月至2020年5月确诊的137例中老年手部慢性湿疹患者临床资料和斑贴试验结果。结果:137例患者,男39例,女98例,年龄40~77岁,平均52.69岁;斑贴试验阳性率72.26%,阳性率与性别无关;40岁组、50岁组阳性率均显著高于60岁以上组;阳性率前6名的变应原依次为:氯化镍、氯化钴、碘丙炔正丁胺甲酸酯、硫酸铜、十二醇硫酸钠、硫酸镍;女性患者氯化镍和十二醇硫酸钠的阳性率高于男性患者。结论:斑贴试验有助于明确中老年患者手部慢性湿疹的致敏原。Objective:To summarize the common allergens of hand chronic eczema in middle-aged and elderly patients.Methods:The clinical data and patch test results of 137 middle-aged and elderly patients with chronic hand eczema from January 2019 to May 2020 were retrospectively analyzed.Results:There were 137 patients(39 males and 98 females)aged 40~77 years old,with an average of 52.69.The overall positive rate of patch test was72.26%,which was not related to gender.The positive rate of patch test in 40 years old group and 50 years old group was significantly higher than in over 60 years old group.The top-six antigens in positive rates were nickel chloride,cobalt chloride,iodopropynyl butylcarbamate,copper sulfate,sodium dodecyl sulfate,nickel sulfate.The positive rates of nickel chloride and sodium dodecyl sulfate in female patients were higher than in male patients.Conclusion:Patch test is helpful to identify the allergens of chronic hand eczema in middle-aged and elderly patients.
