摘 要:目的比较腓骨髓内钉与切开复位钉板系统治疗腓骨远端骨折的疗效。方法回顾性分析2017年10月至2019年10月于锦州医科大学附属第一医院采用手术治疗腓骨骨折的患者,按照纳入、排除标准筛选病例,最终A组髓内钉组25例纳入研究、B组钉板内固定组35例纳入研究。比较两组入院至行腓骨手术的时间,手术时长,腓骨术口总长度,AOFAS评分,并发症的发生。结果A、B两组在术前等待时间、骨折愈合时间,腓骨总切口长度上,差异有统计学意义(P>0.05)。A组优于B组,髓内钉组术前等待时间更短,骨折愈合更快,腓骨总切口长度更短。A、B组在手术时长上,差异无统计学意义(P>0.05)。结论腓骨髓内钉治疗腓骨远端骨折有着良好的生物力学固定强度,治疗效果佳,相比传统切开复位钉板系统方案,术前等待时间更短,腓骨骨折愈合更快,术后并发症的发生率更低。Objective To compare the efficacy of peroneal nailing and open reduction nail plate system in the treatment of distal fibular fractures.Methods The patients who underwent surgical treatment for fibula fractures in the First Affiliated Hospital of Jinzhou Medical University from October 2017 to October 2019 were retrospectively analyzed.The patients were screened according to the inclusion and exclusion criteria.Finally,25 patients in the group A with intramedullary nail and 35 patients in the group B with nail plate internal fixation were included in the study.The time from admission to fibular surgery,operation duration,total length of fibular incision,AOFAS score and incidence of complications were compared between the two groups.Results There were significant differences between group A and group B in preoperative waiting time,fracture healing time and total incision length of fibula(P<0.05).There was no statistical significance in the duration of operation between group A and B(P>0.05).Conclusion Intra-fibular nailing in the treatment of distal fibular fractures has good biomechanical fixation strength and good therapeutic effect.Compared with the traditional open reduction nail plate system,the waiting time before surgery is shorter,the healing of fibular fractures is faster and the incidence of postoperative complications is lower.
