云南省2006—2019年孕产妇艾滋病流行特征及趋势分析

摘　　要：目的分析云南省2006—2019年孕产妇艾滋病流行病学特征和趋势,为制定预防艾滋病母婴传播工作策略提供科学依据。方法从国家《预防艾滋病、梅毒和乙肝母婴传播(IPMTCT)管理信息系统》导出2006—2019年孕产妇接受HIV检测及HIV阳性孕产妇个案信息等相关资料,用Excel 2013对数据进行整理,SPSS 22.0对数据进行统计描述。结果 2006—2019年期间云南省孕产妇HIV阳性检出率在0.21%~0.28%之间,将云南省2010年人口普查数据作为标准组进行标化后阳性检出率在0.14%~0.17%之间。在检出的阳性孕产妇中往年确认阳性并重复怀孕比例从2006年5.53%上升到2019年的57.14%;14年间年龄别构成30~39岁组从14.77%上升到47.43%,而20~29岁组从72.73%下降到38.68%,婚姻状况构成再婚从10.61%上升到35.16%,未婚比从1.52%上升到6.62%,而初婚者从81.06%下降到48.87%;边境县(市、区)的孕产妇HIV阳性检出率高于内地的县(市、区)。结论 14年间云南省孕产妇粗HIV阳性检出率总体呈上升趋势,校正阳性检出率相对平稳,原阳再孕比例呈逐年上升趋势。HIV感染孕产妇年龄分布呈30~39岁组上升,20~29岁组下降的趋势,婚姻状况分布呈再婚和未婚上升,初婚者下降的趋势。HIV阳性孕产妇地区分布呈边境县(市、区)的高于内地县(市、区)的趋势。Objective To analyze the epidemiological feature and tendency of HIV infected pregnant women from 2006 to 2019 in Yunnan Province, and to provide scientific evidence to formulate work strategy of prevention mother-to-child transmission(PMTCT). Methods Related data of pregnant women accepting HIV testing and HIV infected maternal card was exported from national information management system. Excel 2013 and SPSS 20.0 were used to statistics description. Results The rate of Maternal HIV antibody positive was between 0.21%~0.28% from 2006 to 2019 in Yunnan province. Taking the 2010 census data of Yunnan Province as the standard group for standardization, the positive was between 0.14% and 0.17%.The rate of Maternal repeated pregnancy, past HIV positive, who came from HIV infected pregnant women from 5.53% to 57.14% during 2006 to 2019.The proportion of age and marital status was transformation during 14 years, 30-39 age group from 14.77% to 47.43%,but 20-29 age group from 72.73% to 38.68%,and remarriage women from 10.61% to 35.16%, single women 1.52% to 6.62%, but first marriage from 81.06% to 48.87%. The rate of HIV infected pregnant women in border counties was higher than in not border counties. Conclusion The rate of maternal HIV positive detection has remained fairly flat during 14 years in Yunnan province.Corrected positive detection rate is relatively stable.The rate of Maternal repeated pregnancy, past HIV positive, increased yearly. HIV infected pregnant women age distribution trended 30~39 age group uptrend, 20~29 age group downtrend. HIV infected pregnant women marital status distribution trended remarriage and single women uptrend, first marriage women downtrend. HIV infected pregnant women regional distribution trended border counties was higher than not border counties.
