摘  要:目的探究阿格列汀联合不同胰岛素强化方案对2型糖尿病患者微小RNA-144(microRNA-144,miR-144)、微小RNA-142(microRNA-142,miR-142)表达的影响。方法选取我院接诊的2型糖尿病患者106例,按随机数字表法分为两组,每组各53例。两组均采用阿格列汀进行治疗,胰岛素泵组采用胰岛素泵进行治疗,基础治疗组采用皮下胰岛素进行治疗。采用血糖测试仪检测空腹血糖(fasting blood glucose,FBG)、餐后2 h血糖(2 h blood glucose,2 hBG)水平;Hb201+快速血红蛋白检测仪检测糖化血红蛋白(hemoglobin,HbAlc)水平;酶联免疫吸附法检测C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、白细胞介素6(interleukin-6,IL-6)水平;实时荧光定量聚合酶链反应法检测miR-144、miR-142表达水平。观察比较两组治疗总费用、治疗效果以及不良反应发生情况。结果治疗后两组FBG、2 hBG、HbAlc水平均低于治疗前,胰岛素泵组FBG、2 hBG、HbAlc水平、治疗总费用低于基础治疗组(P<0.05),治疗后两组炎性因子水平低于治疗前,胰岛素泵组炎性因子水平低于基础治疗组(P<0.05),治疗后两组miR-144表达低于治疗前,miR-142表达高于治疗前,胰岛素泵组miR-144表达低于基础治疗组,miR-142表达高于基础治疗组(P<0.05),胰岛素泵组治疗总有效率高于基础治疗组(P<0.05),两组总不良反应发生率比较差异无统计学意义(P>0.05)。结论阿格列汀联合胰岛素泵可有效缓解2型糖尿病患者临床症状,降低miR-144表达及并发症的发生率,升高miR-142表达,缩短治疗周期,减少治疗费用,效果显著。Objective To explore the effects of alogliptin combined with different insulin intensive regimens on the expression of microRNA-144(miR-144)and microRNA-142(miR-142)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 106 patients with T2DM treated in our hospital were selected as the research subjects,and they were divided into insulin pump group(n=53)and basic treatment group(n=53)according to the random number table method.Both groups were treated with alogliptin.In addition,the insulin pump group was treated with an insulin pump,and the basic treatment group was treated with subcutaneous insulin.A blood glucose meter was used to detect fasting blood glucose(FBG)and 2 h postprandial blood glucose(2 hBG)levels.Hb201+fast hemoglobin detector was employed to detect hemoglobin(HbAlc)levels.Enzyme-linked immunosorbent assay was used for detection of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6)levels.Real-time fluorescent quantitative PCR method was applied to detect expression levels of miR-144 and miR-142.The total treatment cost,therapeutic effect and occurrence of adverse reactions between two groups were observed and compared.Results After treatment,the levels of FBG,2 hBG and HbAlc in the two groups were lower than those before treatment,and the levels of FBG,2 hBG and HbAlc in the insulin pump group and the total treatment cost were lower than those in the basic treatment group(P<0.05).After treatment,the levels of inflammatory factors in the two groups were lower than those before treatment,and the levels of inflammatory factors in the insulin pump group were lower than those in the basic treatment group(P<0.05).After treatment,the expression of miR-144 in the two groups was lower than that before treatment,while the expression of miR-142 was higher than that before treatment.The expression of miR-144 in the insulin pump group was lower than that in the basic treatment group,while the expression of miR-142 was higher than that in the basic treatment group(P<0
