摘  要:目的观察菊粉型果聚糖干预对妊娠期糖尿病患者糖脂代谢的影响。方法选择2019年在南京市妇幼保健院定期产检的妊娠期糖尿病孕妇100例,随机分为两组,对照组进行常规医学营养治疗,试验组在此基础上给予4周菊粉型果聚糖补充(10g/d),比较治疗前后空腹血糖(FBG)、糖化血红蛋白(HbA1c)及血脂变化情况。结果干预前,对照组膳食纤维摄入量为(15.92±3.64)g/d,干预组为(16.32±3.57)g/d,差异无统计学意义(t=-0.07,P=0.94);干预后,干预组增加至(27.13±5.31)g/d,差异有统计学意义(t=-6.61,P<0.01),且明显高于对照组[(18.33±2.76)g/d](t=-5.36,P<0.01)。干预前,两组FBG、HbA1c水平差异均无统计学意义(P值均>0.05);治疗4周后,两组FBG、HbA1c水平均明显下降,差异均有统计学意义(P值均<0.05),但干预组HbA1c降幅较对照组更大(t=6.29,P<0.01)。干预前,两组三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白(HDL-C)和低密度脂蛋白(LDL-C)水平差异无统计学意义(P值均>0.05);治疗4周后,对照组TG、TC和LDL-C均显著升高(P值均<0.05),干预组仅TC显著升高(P<0.05),其他指标无明显变化(P值均>0.05),且干预组TG、TC、LDL-C水平均低于对照组,差异均有统计学意义(t=3.73、2.20、4.95,P值均<0.05)。结论在常规医学营养治疗的同时补充菊粉型果聚糖,对改善妊娠期糖尿病孕妇糖脂代谢具有重要意义。Objective To explore the effects of inulin-type fructans on the glycolipid metabolism of female gestational diabetes mellitus patients.Methods A total of 100 gestational diabetic who take routine examinations in Obsterics and Gynecology Hospital Affiliated to Nanjing Medical University from Jun to Dec 2019 were selected and randomly divided into 2 groups.The control group received conventional medical nutrition therapy(MNT),and the experimental group received inulintype fructans supplement(10 g/d)for 4 weeks on the basis of conventional MNT.Fasting blood glucose(FBG),glycosylated hemoglobin(HbA1 c)and blood lipid before and after treatment were compared between the 2 groups.Results Before treatment,the dietary fiber intake levels were(15.92±3.64)g/d and(16.32±3.57)g/d for the control group and experimental group,respectively,with no significant difference(t=-0.07,P=0.94);after treatment,the dietary fiber intake level for experimental group increased to(27.13±5.31)g/d,the difference was statistically significant(t=-6.61,P<0.01),and it was significantly higher than that of the control group[(18.33±2.76)g/d],with significant difference(t=-5.36,P<0.01).Before treatment,there was no significant difference among levels of FBG and HbA1 cbetween 2 groups(all P>0.05);after 4 weeks of treatment,the levels of FBG and HbA1 c in 2 groups decreased significantly(all P<0.05),the decrease of HbA1 cin the experimental group was greater than that in the control group,with significant difference(t=6.29,P<0.01).Before treatment,there was no significant difference among levels of triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL-C)and low density lipoprotein(LDL-C)between the 2 groups(all P>0.05).After 4 weeks of treatment,TG,TC and LDL-C in the control group and TC level in experimental group increased significantly(all P<0.05);while other indexes in experimental group did not change significantly(all P>0.05).The levels of TG,TC and LDL-C in the experimental group were lower than those in the control group,with
