腹腔镜保留输卵管浆膜层的输卵管切除术对输卵管妊娠患者卵巢功能的影响

摘　　要：目的分析腹腔镜保留输卵管浆膜层的输卵管切除术对输卵管妊娠患者卵巢功能的影响。方法选取2018年1月-2019年1月义乌復元医院妇科收治的输卵管妊娠患者61例随机分为A组(观察组)33例行腹腔镜下保留输卵管表面浆膜层的输卵管切除术,B组(对照组)28例行常规腹腔镜下患侧输卵管切除术,研究两组患者术前抗苗勒管激素(AMH)、术后1个月AMH、术后3个月AMH变化情况,术后1个月、术后3个月的基础窦卵泡计数情况。结果A组患者术后1个月、术后3个月AMH显著高于B组患者,差异有统计学意义(P<0.05)。A组患者术后1个月、术后3个月窦卵泡数均显著高于B组患者,差异有统计学意义(P<0.05)。结论输卵管妊娠患者行腹腔镜下输卵管切除手术可影响患者卵巢功能,因此可选择保留输卵管浆膜层的输卵管切除术,以保护卵巢功能。Objective To analyze the effect of preserving serosal layer of Fallopian tube laparoscopic salpingectomy on ovarian function of tubal pregnant patients.Methods From January 2018 to January 2019,61 patients with tubal pregnancy were selected from Department of Gynecology in Yiwu Fuyuan Hospital,then they were randomly divided into group A(33 cases)and group B(28 cases),the patients in group A were treated by preserving serosal layer of Fallopian tube laparoscopic salpingectomy,the patients in group B were treated by routine laparoscopic salpingectomy.The levels of preoperative anti-Müllerian hormone(AMH),the changes of AMH at one and three months after surgery,basal sinus follicle counts at one and three months after surgery in the two groups were researched.Results The levels of AMH at one and three months after surgery in group A were statistically significantly higher than those in group B(P<0.05),basal sinus follicle counts at one and three months after surgery in group A were statistically significantly higher than those in group B(P<0.05).Conclusion Laparoscopic salpingectomy may affect ovarian function of patients with tubal pregnancy,preserving serosal layer of Fallopian tube laparoscopic salpingectomy can be chosen to protect ovarian function.
