摘  要:目的分析术中超声辅助切除颅脑深部病灶的应用价值。方法选择梅州市人民医院2018年1月—2020年5月行术中实时超声定位辅助显微外科手术治疗的颅脑深部病灶患者25例,对其临床资料进行回顾性分析,评价术中超声精确定位病变指导外科手术操作的临床价值。结果25例颅脑深部病灶行术中实时超声定位辅助显微外科手术治疗的切除率为96.00%。15例良性病灶术中实时超声定位引导下一次性成功切除率为100%,10例恶性病灶术中实时超声定位引导下一次性成功切除率为90.00%。良恶性病灶术中超声声像图均以高回声为主要特点,其中良性病变者术中超声声像图发现异常血流信号4例,恶性病变者术中超声声像图特征主要为边界不清晰、周围水肿带、病灶内有血流信号。结论术中实时超声定位辅助显微外科手术治疗颅脑深部病灶能够对病灶进行定位诊断,进一步明确病灶的具体情况,指导外科手术操作,对准确切除病灶、提高手术成功率具有积极意义。Objective To analyze the application value of intraoperative ultrasound assisted resection of deep brain lesions.Methods From January 2018 to May 2020,25 patients with deep brain lesions who underwent intraoperative real-time ultrasound localization assisted microsurgery in our hospital were selected.The clinical data were retrospectively analyzed to evaluate the important value of intraoperative ultrasound precise localization of lesions in guiding surgical operation.Results The resection rate of 25 cases of deep brain lesions treated with intraoperative real-time ultrasound localization assisted microsurgery was 96.00%.The one-time successful resection rate was 100%in 15 cases of benign lesions and 90%in 10 cases of malignant lesions.In benign lesions,abnormal blood flow signals were found in 4 cases,while in malignant lesions,the main characteristics of intraoperative ultrasound images were unclear boundary,surrounding edema zone and blood flow signals in lesions.Conclusion Intraoperative real-time ultrasound localization assisted microsurgery in the treatment of deep brain lesions can make localization diagnosis of the lesions,further clarify the specific situation of the lesions,guide the surgical operation,and have positive significance for accurate resection of the lesions and improving the success rate of operation.
