北京市顺义区儿童乙型肝炎疫苗5年追访效果评价

摘　　要：目的评价北京市顺义区新生儿按照“0-1-6”全程接种10μg重组乙肝疫苗后的乙肝表面抗体(HBsAb)阳性率、抗体浓度以及5年后的免疫效果。方法按照单纯随机抽样原则,2013年抽取顺义区某二级医院165名完成“0-1-6”乙肝疫苗接种程序的7~12个月龄儿童进行采血检测,5年后对随访到的儿童再次进行静脉采血。结果2013年调查的165名儿童中HBsAb阳性率为99.39%(164/165),乙肝表面抗体几何平均浓度(HBsAb GMC)为3664.06 mIU/mL。2018年随访调查的84名儿童中HBsAb阳性率77.38%(65/84),HBsAb GMC为141.69 mIU/mL。2013—2018年检测儿童的GMC减少程度,差异有统计学意义(P<0.05)。结论顺义区新生儿完成乙肝疫苗免疫程序接种后HBsAb阳性率很高,5年后HBsAb GMC随着免疫时间的延长而下降明显,但仍能维持于较高水平。Objective To evaluate the antibody positive rate,Geometric Mean Concentration and hepatitis B vaccine immune effect after five years of the newborn who have been completed the"0-1-6"hepatitis B vaccination immune procedure.Methods Simple random method was adopted,165 children aged 7 to 12 months were randomly selected who have been completed the"0-1-6"hepatitis B vaccination immune procedure from a Second-class Hospital in Shunyi District,and blood examination were performed in 2013,and blood examination were repeated after five years.Results 99.39%(164/165)of all children investigated in the survey of 2013 turned positive,among the 84 children investigated in the survey of 2018,and 77.38%(64/84)of children turned positive.Children's hepatitis B surface antibody Geometric Mean Concentration of 2013 was 3664.06 mIU/ml,and children's hepatitis B surface antibody Geometric Mean Concentration of 2018 was 141.69mIU/ml.Degree of antibody titer reduction had statistical significance.Conclusion Newborns in Shunyi District,Beijing,had a higher positive rate of antibody after completing three doses of hepatitis B vaccination.Five years later children′s hepatitis B surface geometric mean concentration decreased significantly with time,and still remained at a high level.
