摘　　要：目的:研制用于战场环境下的新型便携式战创伤性血(气)胸及多发肋骨骨折救治包,降低伤员伤亡率。方法:依据现代战争战场环境因素及战创伤特点,设计包含开放性创伤模块、血(气)胸救治模块和多发肋骨骨折救治模块的战创伤性血(气)胸及多发肋骨骨折救治包。选取22只比格犬进行动物实验,按照随机数表法将其分为实验组和对照组,每组11只,对照组采用传统血气胸救治方法,实验组采用便携式血气胸救治包内物品进行救治。对比分析两组手术时间、术后1~6 h每小时引流量、最后拔管时间、引流管固定牢固度以及术后生存情况。结果:实验组的救治时间为7.56 min,对照组为11.95 min,其差异有统计学意义(t=13.53,P<0.05);两组术后1~6 h每小时引流量、最后拔管时间、引流管固定牢固度以及术后生存情况差异均无统计学意义。结论:新型便携式战创伤性血(气)胸及多发肋骨骨折救治包在完成简单操作和节省时间要求的同时,能保证救治质量,适合在战场推广应用。Objective:To develop a new portable war-traumatic hemopneumothorax and multiple rib fracture treatment kit used in battlefield environment to reduce the casualty rate of the wounded.Methods:Based on the environmental factors of modern warfare and the characteristics of war trauma,designed a war trauma hemopneumothorax and multiple rib fracture treatment kit included open trauma module,hemopneumothorax treatment module and multiple rib fracture treatment module.22 beagle dogs for animal experiment were selected and divided into experimental group and control group according to the random number table method,11 in each group.The control group used traditional hemopneumothorax treatment method,and the experimental group used the portable hemopneumothorax treatment kit.The operation time,drainage volume per hour 1 to 6 hours after surgery,final extubating time,firmness of drainage tube fixation and postoperative survival status were compared and analyzed.Results:The treatment time of the experimental group was 7.56 min and that of the control group was 11.95 min,the difference was statistically significant(t=13.53,P<0.05).There were no statistically significant differences in drainage volume per hour 1 to 6 hours after surgery,final extubating time,firmness of drainage tube fixation and postoperative survival status.Conclusion:The new portable war-traumatic hemopneumothorax and multiple rib fracture treatment kits can ensure the quality of treatment,at the same time,satisfying the requirements of simple operation and saving time,and is suitable for popularization and application on the battlefield.
