踝关节撞击综合征的关节镜治疗

摘　　要：目的:探讨关节镜技术在踝关节撞击综合征中的应用及技术要点。方法:对2016年7月—2019年7月经保守治疗无效的踝关节撞击综合征患者共22例进行关节镜检查、关节腔清理术。术后踝关节腔放置引流1 d,加压包扎4 d,10 d拆线。术后按自定的康复措施进行锻炼,对患者术前、术后1个月及3个月踝关节功能进行Mazu评分。结果:所有患者均获随访,切口Ⅰ期愈合,无感染,1例患者术中足背动脉损伤,予小切口分离足背动脉后缝合1针后通畅。所有患者疼痛症状均较术前明显缓解,踝关节屈伸功能明显好转。术前、术后1个月及3个月踝关节Mazu评分分别为(55.0±8.2)、(85.5±8.7)和(91.7±7.9)分。结论:踝关节撞击综合征是踝关节常见疾病,早期诊断并明确撞击原因是确保疗效及缓解并发症的前提。踝关节镜手术由于创伤小、治疗彻底、恢复较快等优点,已成为治疗踝关节撞击综合征的首选术式。Objective:To explore the methods and techniques of arthroscopy in the treatment of ankle impingement syndrome.Methods:A total of 22 patients who were diagnosed with ankle impingement syndrome from July 2016 to July 2019 and who failed conservative treatment were undergone the arthroscopic operation and joint cavity debridement.After surgery,the ankle joint cavity was placed and drained for 1 day,compression bandaged for 4 days,and suture removed 10 days.After the operation,the patients were exercised according to the self-determined rehabilitation exercise measures,and the patients′ankle joint function was evaluated by Mazu score system before the operation,1 month and 3 months after the operation.Results:All patients were followed up.The incisions healed by first intention and there was no infection.One patient was injured in the dorsal foot artery during the operation and sutured with one stitch.The pain symptoms of all patients were significantly relieved compared with preoperatively,and the ankle flexion and extension function were significantly improved.The ankle Mazu scores before the operation,1 month,and 3 months after operation were(55.0±8.2),(85.5±8.7)and(91.7±7.9)points,respectively.Conclusion:Ankle impingement syndrome is a common disease of the ankle joint.Early diagnosis and identification of the cause of impingement is a prerequisite to ensure the efficacy and alleviate complications.Arthroscopic operations have become the first choice for the treatment of this type of disease due to its advantages such as less trauma,thorough treatment,and faster recovery.
