分析球囊宫颈扩张器配合人工破膜应用于足月孕妇引产的临床效果

摘　　要：目的对球囊宫颈扩张器配合人工破膜应用于足月孕妇引产的临床效果进行分析。方法选取2018年12月—2019年12月该院的70例足月引产孕妇,随机分为参照组和实验组,每组35例。参照组孕妇接受球囊宫颈扩张器配合缩宫素静滴引产,实验组孕妇接受球囊宫颈扩张器配合人工破膜及缩宫素静滴引产,对两组引产效果进行分析。结果干预后6、12 h实验组Bishop评分(6.02±1.17)分、(7.28±1.19)分高于参照组的(4.36±1.14)分、(6.03±1.08)分,差异有统计学意义(t=6.012、4.602,P<0.05);实验组自然分娩率94.29%高于参照组77.14%,剖宫产率5.71%低于参照组22.86%,差异有统计学意义(χ^(2)=4.154,P<0.05),实验组产程出血量(109.45±30.27)mL、产后出血量(201.65±13.17)mL、均少于参照组,新生儿Apgar评分(9.52±0.43)分高于参照组,差异有统计学意义(t=5.788、6.692、9.659,P<0.05);两组新生儿体重差异无统计学意义(t=0.309,P>0.05)。结论球囊宫颈扩张器配合人工破膜及缩宫素静滴引产可以有效提高孕妇自然分娩率,减少产程及产后出血量,改善母婴结局。Objective To analyze the clinical effect of balloon cervical dilator combined with artificial membrane rupture for induction of labor in term pregnant women.Methods A selection of 70 full-term pregnant women in this hospital from December 2018 to December 2019 were randomly divided into a reference group and an experimental group,with 35 cases in each group.Pregnant women in the reference group received a balloon cervical dilator combined with intravenous oxytocin to induce labor,and pregnant women in the experimental group received a balloon cervical dilator combined with artificial rupture and intravenous oxytocin to induce labor.The effects of the two groups were analyzed.Results At 6 and 12 hours after the intervention,the Bishop score of the experimental group was(6.02±1.17)points and(7.28±1.19)points higher than the reference group's(4.36±1.14)points and(6.03±1.08)points,the difference was statistically significant(t=6.012,4.602,P<0.05);the natural delivery rate of the experimental group was 94.29% higher than the reference group 77.14%,the cesarean section rate was 5.71% lower than the reference group 22.86%,the difference was statistically significant(χ^(2)=4.154,P<0.05),the experimental group’s bleeding during labor was(109.45±30.27)mL The amount of postpartum hemorrhage(201.65±13.17)mL was less than that of the reference group,and the Apgar score(9.52±0.43)points of the newborn was higher than that of the reference group,the difference was statistically significant(t=5.788,6.692,9.659,P<0.05);the weight of the two groups of newborns The difference was not statistically significant(t=0.309,P>0.05).Conclusion Balloon cervical dilator combined with artificial membrane rupture and intravenous infusion of oxytocin can effectively increase the natural delivery rate of pregnant women,reduce labor and postpartum hemorrhage,and improve maternal and infant outcomes.
