摘 要:目的探讨胫骨横向骨搬运技术治疗糖尿病足的临床疗效。方法在2019年6月—2020年2月间抽选该院收治的142例糖尿病足患者作为研究对象,按治疗方式的不同将其分为对比组(采用皮瓣手术治疗)和研究组(采用胫骨横向骨搬运治疗),每组71例。比较两组治疗前、治疗后的血管内皮生长因子水平、数字评分法(NRS)评分、踝肱指数(ABI)、密歇根神经病变筛查量表(MNSI)评分,并对比两组的足部创面愈合时间、截肢率、并发症发生率。结果治疗前,两组的血管内皮生长因子水平、NRS评分、ABI、MNSI评分差异无统计学意义(P>0.05)。治疗后,研究组的血管内皮生长因子水平、ABI高于对比组,而NRS评分、MNSI评分低于对比组,差异有统计学意义(P<0.05);研究组的足部创面愈合时间短于对比组,差异有统计学意义(P<0.05);研究组的截肢率、并发症发生率明显低于对比组,差异有统计学意义(P<0.05)。结论为糖尿足患者实施胫骨横向骨搬运技术治疗,可提升其体内的血管内皮生长因子,促进创面愈合,改善疾病,效果显著,且并发症风险低,最终能进一步减少截肢几率。Objective To investigate the clinical effect of tibia lateral bone transport technology in the treatment of diabetic foot.Methods From June 2019 to February 2020,142 diabetic foot patients admitted to the hospital were selected as the research objects.According to the different treatment methods,they were divided into a comparison group(treatment with flap surgery)and a study group(treatment with flaps tibia transverse bone transport treatment),71 cases in each group.Compare the levels of vascular endothelial growth factor,Numerical Score(NRS)score,Ankle Brachial Index(ABI),Michigan Neuropathy Screening Inventory(MNSI)scores before and after treatment between the two groups,and compare the foot wounds healing time,amputation rate,and complication rate of the two groups.Results Before treatment,there was no statistical difference in the levels of vascular endothelial growth factor,NRS score,ABI,and MNSI score between the two groups(P>0.05).After treatment,the levels of vascular endothelial growth factor and ABI of the study group were higher than those of the control group,while the NRS score and MNSI score were lower than those of the control group,and the difference was statistically significant(P<0.05);the foot wound healing time of the study group was shorter than that of the control group,and the difference was statistically significant(P<0.05);The amputation rate and complication rate of the study group were significantly lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of tibia lateral bone transport technology for patients with diabetic foot can increase the vascular endothelial growth factor in their body,promote wound healing,improve the disease,the effect is significant,and the risk of complications is low,and ultimately can further reduce the chance of amputation.
