老年股骨粗隆间骨折应用PFNA联合PFN内固定治疗临床有效率分析

摘　　要：目的分析老年股骨粗隆间骨折应用PFNA联合PFN内固定治疗临床有效率。方法随机选择2018年3月—2020年2月期间该院收治的老年股骨粗隆间骨折患者74例,利用抽签法平均分两组,对照组(37例,PFN治疗)和试验组(37例,PFN治疗础上联合PFNA治疗),比较临床疗效、并发症率、髋关节功能恢复情况、日常生活能力和疼痛情况。结果试验组治疗总有效率(94.60%)高于对照组78.38%,并发症率(2.70%)于低对照组21.62%,差异有统计学意义(χ^(2)=4.163、4.554,P<0.05);术后试验组Harris评分(81.67±3.77)分、ADL评分(67.11±3.74)分高于对照组(76.23±3.12)分、(60.37±3.69)分,NRS评分(3.07±1.07)分低于对照组(4.43±1.12)分,差异有统计学意义(t=6.762、7.803、5.341,P<0.05)。结论针对老年股骨粗隆间骨折患者,PFNA联合PFN内固定更利于患者髋关节功能恢复,效果突出,且利于提升患者生活能力,缓解其痛感,同时安全性较高,值得推荐。Objective To analyze the clinical effectiveness of PFNA combined with PFN internal fixation in the treatment of intertrochanteric fractures in the elderly.Methods From March 2018 to February 2020,random sdection 74 elderly patients with intertrochanteric fractures of the femur who were admitted to the hospital during this period were randomly selected and divided into 2 groups by lottery.The control group(37 cases,PFN treatment)and the experimental group(37 cases,combined with PFNA treatment based on PFN treatment),to compare the clinical efficacy,complication rate,hip function recovery,ability of daily living and pain.Results The total effective rate(94.60%)in experimental group was higher than that in control group(78.38%),and the complication rate(2.70%)was 21.62%in low control group,the difference was statistically significant(χ^(2)=4.163,4.554,P<0.05).Harris score(81.67±3.77)points and ADL score(67.11±3.74)points in the experimental group were higher than those in the control group(76.23±3.12)points and(60.37±3.69)points,and NRS score(3.07±1.07)points was lower than those in the control group(4.43±1.12)points,the difference was statistically significant(t=6.762,7.803,5.341,P<0.05).Conclusion For elderly patients with femoral intertrochanteric fractures,PFNA combined with PFN internal fixation is more conducive to the recovery of hip joint function,the effect is outstanding,and it is beneficial to improve the patient's living ability and relieve their pain.At the same time,it is safe and worthy of recommendation.
