疏肝补肾法辨治腰痛

摘　　要：随着社会的发展、生活质量的提高及人们久坐等生活习惯的改变,腰痛的发病率逐渐升高。腰痛不仅给患者带来疼痛难忍的不适感,还影响其生活质量,进一步影响其学习和工作,给患者家庭和社会带来不良影响。西医认为腰痛主要是与腰肌劳损、腰椎间盘突出等腰部病变有关,治疗上主要采取对症治疗为主,止痛效果明显,但易复发,严重影响其临床疗效。中医历史悠久,辨证论治思维贯穿疾病治疗的整个过程,对多种疾病治疗效果显著。本人从事临床工作30余年,对腰痛治疗颇有心得,认为腰痛除与常见的寒湿、湿热等有关外,还与肝郁有关,同时结合“腰者,肾之腑也”,故本人认为腰痛治疗可以从补肾疏肝入手治疗,并使用疏肝补肾汤治疗每获疗效。本文总结归纳了腰痛的病因病机,并从肝肾的生理病理关系阐述腰痛的病因病机,从肝肾论治腰痛的处方用药及临床应用。With the development of the society,the improvement of the quality of life and the change of people's sedentary living habits,the incidence of low back pain increases gradually.Low back pain not only brings pain and discomfort to patients,but also affects their quality of life,further affects their study and work,and brings adverse effects to patients'families and society.Western medicine believes that lumbago is mainly related to lumbar diseases such as lumbar muscle strain and lumbar intervertebral disc herniation.Symptomatic treatment is mainly adopted in treatment,which is with obvious analgesic effect but it is easy to relapse,seriously affecting its clinical efficacy.Traditional Chinese medicine(TCM)has a long history,and the thinking of treatment based on syndrome differentiation runs through the whole process of disease treatment,which has a significant effect on the treatment of many diseases.I have been engaged in clinical work for more than 30 years,and have gained a lot of experience in the treatment of low back pain.I believe that low back pain is not only related to common cold and dampness,dampness and heat,but also related to liver depression.At the same time,combined with"the waist is the kidney organ",I believe that the treatment of low back pain can start with the treatment of tonifying the kidney and soothing the liver,and apply Shugan Bushen decoction to gain curative effect.This paper summarizes the etiology and pathogenesis of low back pain,and expounds the etiology and pathogenesis of low back pain from the physiological and pathological relations of liver and kidney,and discusses the prescription medication and clinical application of low back pain treatment based on liver and kidney.
