雷火灸热敏穴联合间歇性清洁导尿干预神经源性膀胱的护理研究

摘　　要：目的:探讨雷火灸热敏穴联合间歇性清洁导尿干预神经源性膀胱的临床疗效。方法:选取2017年5月-2019年1月期间于东莞市中医院住院治疗的神经源性膀胱患者120例为研究对象,随机分为对照组和实验组,每组各60例。对照组予以神经源性膀胱一般治疗及间歇性清洁导尿处理,实验组在此基础上实施雷火灸热敏穴治疗。对比干预前后两组患者尿流动力学指标、膀胱功能及患者总体满意度情况。结果:经过治疗,实验组患者最大膀胱容积、充盈期逼尿肌压力及逼尿肌漏尿点压均显著高于对照组,漏尿次数显著少于对照组,差异有统计学意义(P<0.05)。干预4周后,实验组膀胱容量较对照组明显增加,残余尿量较对照组明显减少,差异具有统计学意义(P<0.05)。干预后实验组患者的症状积分显著低于对照组,差异具有统计学意义(P<0.05)。实验组患者总体满意度显著高于对照组,差异具有统计学意义(P<0.05)。结论:雷火灸热敏穴联合间歇性清洁导尿干预神经源性膀胱可显著改善膀胱功能,提高患者的总体满意度。Objective:To investigate the clinical effects of Leihuo-moxibustion(雷火灸)at thermosensitive acupoints plus intermittent cleaning and catheterization on neurogenic bladder.Methods:120 cases were randomly divided into the control group and the experimental group,with 60 cases in each group.The control group received general treatment for neurogenic bladder,and intermittent cleaning and catheterization.The experimental group was given Leihuo-moxibustion at thermosensitive acupoints more.Results:After treatment,the maximum bladder volume,detrusor pressure at the filling stage and detrusor urine leakage point pressure in the experimental group were significantly higher than those in the control group,the frequency of urine leakage was significantly lower than the control group,with statistically significant differences(P<0.05).After 4 weeks of intervention,the experimental group showed a significant increase in bladder volume and a significant decrease in residual urine volume compared with the control group(P<0.05).After intervention,the symptom scores in the experimental group were significantly lower than the control group,with statistically significant differences(P<0.05).The overall satisfaction of patients in the experimental group was significantly higher than the control group,and the difference was statistically significant(P<0.05).Conclusion:Leihuo-moxibustion at thermosensitive acupoints plus intermittent cleaning and catheterization on neurogenic bladder can significantly improve the bladder function of patients,and improve the overall satisfaction of patients.
