益气活血通络汤联合针灸治疗缺血性脑卒中偏瘫患者效果观察

摘　　要：目的:观察益气活血通络汤联合针灸对缺血性脑卒中偏瘫患者的治疗效果。方法:选取2017年2月-2019年11月因缺血性脑卒中偏瘫入院的患者113例为研究对象,随机分为两组,对照组采用常规康复训练法,研究组在此基础上给予益气活血通络汤联合针灸治疗。比较两组临床疗效以及治疗前后患者肢体运动功能、日常生活质量、活动能力、步态运动学水平以及不良反应发生率。结果:治疗后研究组临床总有效率显著高于对照组,差异具有统计学意义(P<0.05)。两组患者治疗前上下肢运动功能Fugel-Meyer评定量表(Fugl-Meyer Assessment,FMA)评分、改良Barthel指数(MBI)、脑卒中专用生活质量量表(SS-QOL)评分、脑卒中神经功能缺损评分量表(National Institutes of Health Stroke Scale,NIHSS)评分的差异无统计学意义(P>0.05)。治疗后研究组上下肢FMA评分、MBI评分、SS-QOL评分均高于对照组,NIHSS评分低于对照组,差异有统计学意义(P<0.05)。治疗前两组患者步速、步长以及步频的差异无统计学意义(P>0.05),治疗后研究组患者步速、步长以及步频均显著高于对照组,差异具有统计学意义(P<0.05)。研究组与对照组不良反应发生率的差异无统计学意义(P>0.05)。结论:益气活血通络汤联合针灸治疗可以明显改善缺血性脑卒中偏瘫患者的神经、肢体运动功能以及生活质量,提高患者的步态动力学水平,且无特殊的不良反应,值得应用。Objective:To explore and observe the therapeutic effects of the Yiqi Huoxue Tongluo decoction(益气活血通络汤)plus acupuncture on hemiplegia patients with ischemic stroke.Methods:113 patients were selected and randomly divided into two groups.The control group took conventional rehabilitation training,and the study group was given the Yiqi Huoxue Tongluo decoction more.Results:After treatment,the total efficiency in the study group was significantly higher than the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the FMA scores,MBI scores,SS-QOL scores,and NIHSS scores between the groups(P>0.05).After treatment,the FMA scales,MBI scores,and SS-QOL scores of the upper and lower limbs in the study group were higher than the control group,and the NIHSS scores in the study group were lower than the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in pace,step length,and step frequency between the groups(P>0.05).After treatment,the pace,step length,and step frequency in the study group were significantly higher than the control group,P<0.05.There was no significant difference in the incidence of adverse reactions between the groups(P>0.05).Conclusion:The Yiqi Huoxue Tongluo decoction plus acupuncture can significantly improve the nerve function,limb motor function and life quality,improve the patients’gait dynamics level,with no special adverse reactions,and is worthy of application.
