摘 要:目的探讨新生儿窒息患儿心电图校正QT离散度(QTcd)与血清钠、钾的相关性。方法纳入新生儿窒息患儿100例,根据病情严重程度分为轻度新生儿窒息组65例和重度新生儿窒息组35例,另选取同期健康新生儿60例作为健康对照组。所有新生儿均给予12导联心电图检查,检测血清钠、钾水平,测定肌钙蛋白I(cTnI)水平,根据心电图检查结果计算QTcd,比较不同组别新生儿cTnI、QTcd、血Na^(+)、血K^(+)水平,采用Spearman线性方程分析QTcd与cTnI、血Na^(+)、血K^(+)之间的相关性。结果重度新生儿窒息组cTnI、QTcd水平高于轻度新生儿窒息组和健康对照组,轻度新生儿窒息组cTnI、QTcd水平高于健康对照组,差异具有统计学意义(F=5831.504,277.169,P<0.05);重度新生儿窒息组血Na^(+)、K^(+)水平低于轻度新生儿窒息组和健康对照组,轻度新生儿窒息组血Na^(+)、K^(+)水平低于健康对照组,差异具有统计学意义(P<0.05)。Spearman线性方程显示,QTcd与cTnI水平呈正相关性(r=0.824,P<0.05),与血Na^(+)、K^(+)水平呈负相关性(r=-0.790,-0.640,P<0.05);血K^(+)与cTnI水平呈负相关性(r=-0.679,P<0.05)。结论血清K^(+)和Na^(+)可作为早期评估新生儿窒息心肌损伤的监测标志物,并且对新生儿窒息损伤严重程度具有一定评估作用。Objective To investigate the correlation between corrected QT dispersion and serum sodium and potassium on electrocardiogram(ECG)of children with neonatal asphyxia.Methods 100 children with neonatal asphyxia admitted to our hospital between March 2018 and December 2019 were enrolled.According to the severity of the disease,the patients were divided into mild neonatal asphyxia group(n=65)and severe neonatal asphyxia group(n=35),and another 60 healthy newborns during the same period were selected as the healthy control group(n=60);All newborns checked a 12-lead ECG and detect serum sodium and potassium levels were detected,and troponin I levels was determined;calculate QTcd based on the results of the ECG;And the cTnI,QTcd,blood Na^(+),blood K^(+)levels of newborns were compared in different groups,and the correlation was analyzed between QTcd and cTnI,blood Na^(+),and blood K^(+)by using Spearman linear equation.Results The levels of cTnI and QTcd in the severe neonatal asphyxia group were higher than those in the mild neonatal asphyxia group and the healthy control group.The cTnI and QTcd levels in the mild neonatal asphyxia group were higher than those in the healthy control group(F=5831.504,277.169,P<0.05);The blood Na^(+)and K^(+)levels in the severe neonatal asphyxia group were lower than those in the mild neonatal asphyxia group and the healthy control group,and the blood Na^(+)and K^(+)levels in the mild neonatal asphyxia group were lower than the healthy control group(F=73.791,144.056,P<0.05);Spearman linear equation shows that QTcd was positively correlated with cTnI levels(r=0.824,P<0.05),and negatively correlated with blood Na^(+)and K^(+)levels(r=-0.790,-0.640,P<0.05);The blood K^(+)and cTnI level was negatively correlated(r=-0.679,P<0.05).Conclusion Serum Na^(+)and K^(+)can be used as monitoring markers for early assessment of neonatal asphyxia myocardial injury,and it has a certain role in evaluating the severity of neonatal asphyxia injury.
