六味地黄丸治疗原发性遗尿症的疗效观察及对血清抗利尿激素的影响

摘　　要：目的:探讨六味地黄丸对原发性遗尿症患儿的临床疗效及可能机制。方法:选取我校附属医院儿科收治的经临床诊断为原发性遗尿症患者60例,随机分为对照组和治疗组,各30例。对照组给予行为治疗,治疗组在行为治疗的基础上口服六味地黄丸,疗程3个月。判定两组的临床疗效,并检测治疗前后白天(9 a.m.)和夜间(11 p.m.)血清抗利尿激素(Antidiuretic hormone,ADH)水平变化,随访6个月统计患者的复发率。结果:与对照组比较,六味地黄丸治疗组总有效率明显升高,复发率明显下降,差异有统计学意义(P<0.01);对照组患者夜间(11 p.m.)ADH水平比白天(9 a.m.)明显减少(P<0.01),但治疗前后夜间(11 p.m.)与白天(9 a.m.)ADH分泌都没有明显差异(P>0.05);治疗组治疗前夜间(11 p.m.)ADH水平比白天(9 a.m.)明显减少(P<0.01),治疗后ADH水平变化不明显(P>0.05),且治疗后夜间(11 p.m.)ADH水平明显高于对照组(P<0.01)。结论:六味地黄丸对原发性遗尿症患儿有明显治疗作用,其机制可能是通过刺激机体ADH分泌。Objective:To discuss the clinical effects of Liuwei Dihuang Wan(六味地黄丸)on primary nocturnal enuresis and its possible mechanisms.Methods:60 cases were randomly divided into the control group and the therapy group,with 30 cases in each.The control group was treated by behavioral therapy,while the therapy group was treated with both Liuwei Dihuang Wan and behavioral therapy,and the treatment courses were 3 months.Results:The total efficiency in the therapy group was obviously increased compared with the control group,but the recurrence was obviously decreased compared with the control group(P<0.01).The ADH level of patients in the control group at night(11 p.m.)was obviously lower than daytime(9 a.m.),but before treatment and after treatment,there was no obvious difference in the ADH secretion at night(11 p.m.)and daytime(9 a.m.)in the control group(P>0.05).The ADH level at night(11 p.m.)was obviously lower than daytime(9 a.m.)in the therapy group before treatment(P<0.01),but after treatment,the difference was not obvious(P>0.05),and the ADH level at night(11 p.m.)in the therapy group was significantly higher than the control group(P<0.01).Conclusion:Liuwei Dihuang Wan shows a significant effect on children with PNE,and its mechanism may be by stimulating ADH secretion in patients.
