纤支镜检查联合气囊漏气实验对颈椎前路手术患者拔除气管导管后气道阻塞的风险评估

摘　　要：对颈椎前路手术患者拔除气管导管后采用纤支镜检查联合气囊漏气实验的气道阻塞风险进行评估。选取佛山市中医院2019年1月1日—12月30日收治的行颈椎前路手术后需进行气管导管拔除的60例患者,采用等量数字随机方式分为常规组和实验组,每组各30例。分别给予经验性拔管和纤支镜检查联合气囊漏气实验,对比分析两种方案对患者气道阻塞风险评估的价值。常规组和实验组的气管导管拔除成功率、气管插管时间、ICU住院时间、呼吸机相关性肺炎发生率以及病死率比较,差异具有统计学意义(P<0.05)。纤支镜检查联合气囊漏气实验可以有效评估颈椎前路手术后气管插管患者导管拔除后发生气道阻塞的风险,整体提高患者的拔管安全性和生存率。The research evaluates the bronchofiberscope examination combined with gasbag air leakage experiment on airway obstruction after the remove of tracheal catheter of patients underwent anterior cervical surgery.60 patients,who need endotracheal tube remove and undergo anterior cervical surgery,from Jan.1 to Dec.30,2019 in Foshan Hospital of Traditional Chinese Medicine are selected,and they are divided into routine group and experiment group according to equivalent number random method,with 30 patients in each group.They receive experiment extubation and bronchofiberscope examination combined with gasbag air leakage experiment respectively.The values of two methods on patient airway obstruction risk evaluation are analyzed.Endotracheal tube remove successful rate,trachea cannula time,ICU hospitalization time,ventilator-associated pneumonia incidence and case fatality rate of routine group and experiment group are compared,with statistically significant difference(P<0.05).Bronchofiberscope examination combined with gasbag air leakage experiment can effectively evaluate the risk of airway obstruction after the remove of tracheal catheter of patients underwent anterior cervical surgery,and integrally improve patient extubation safety and survival rate.
