摘　　要：目的分析H型高血压患者吸烟、戒烟对血清同型半胱氨酸(Hcy)水平的影响,分析吸烟指数、戒烟时间与Hcy水平的相关性。方法对2018年10月至2019年10月在北京老年医院就诊的高血压患者进行Hcy检测,高血压合并Hcy≥10μmol/L者为H型高血压患者,筛选出H型高血压人群587例。对该人群全部进行调查问卷。分析H型高血压患者吸烟、戒烟对Hcy水平的影响,以及吸烟指数、戒烟时间与Hcy的相关性。结果目前吸烟的H型高血压患者Hcy水平明显高于从不吸烟者(P<0.001);已戒烟者的Hcy水平高于从不吸烟者(P<0.05);无论是否戒烟,吸烟指数与Hcy水平均呈正相关(戒烟者r=0.273,P=0.007;仍吸烟者r=0.289,P<0.001);戒烟时间与Hcy水平呈负相关,但差异无统计学意义(r=-0.189,P=0.053)。结论吸烟可明显增加H型高血压患者血清Hcy浓度;吸烟指数越高,Hcy水平越高;Hcy水平与戒烟时间长短无关;应以戒烟为目标减少吸烟。Objective To analyze the influence of smoking and quitting on serum homocysteine(Hcy)level in patients with H-type hypertension,and to analyze the correlation between smoking index,quitting time and Hcy level.Methods Hcy test was conducted among hypertensive patients treated in Beijing geriatric hospital from October 2018 to October 2019.Patients with hypertension and Hcy≥10 mol/L were classified as H-type hypertensive patients.587 H-type hypertensive patients were screened.Questionnaires were conducted on all patients.To analyze the influence of smoking and quitting on Hcy level in patients with H-type hypertension,and the correlation between smoking index,quitting time and Hcy.Results The Hcy level of current smokers with H-type hypertension was significantly higher than that of non-smokers(P<0.001).The Hcy level of ex-smokers was higher than that of non-smokers(P<0.05).Smoking index was positively correlated with Hcy level regardless of smoking cessation(ex-smokers:r=0.273,P=0.007,smokers:r=0.289,P<0.001).There was a negative correlation between smoking cessation time and Hcy level but without statistical significance(r=-0.189,P=0.053).Conclusion Smoking significantly increases the serum Hcy concentration in patients with H-type hypertension.The higher the smoking index,the higher the Hcy level.Hcy level was independent of smoking cessation time.Smoking should be reduced with the aim of quitting smoking.
