药物涂层球囊在冠状动脉支架再狭窄介入治疗中的应用价值

摘　　要：目的探讨冠状动脉支架内再狭窄患者中运用药物涂层球囊(DCB)处理的有效性及安全性。方法对60例经冠脉造影证实冠状动脉内支架再狭窄患者,36例予以药物涂层球囊治疗,24例予以药物洗脱支架(DES)治疗,随访6个月并复查冠脉造影进行对比。结果两组患者基线资料上差异无统计学意义(P>0.05);两组患者术后即刻药物洗脱支架组残余狭窄优于药物涂层球囊组,差异有统计学意义(P<0.05);两组6个月随访复测冠脉造影显示两组患者靶病变血管再狭窄率、靶病变血管再次血运重建和主要不良事件(MACE)差异无统计学意义(P>0.05)。两组患者术后6个月出血事件差异无统计学意义(P>0.05)。结论药物涂层球囊法应用于治疗冠状动脉支架内再狭窄患者是安全有效的。Objective To explore the effectiveness and safety of drug-coated balloon(DCB)treatment in patients with coronary artery in-stent restenosis.Methods Among 60 patients with coronary stent restenosis confirmed by coronary angiography,36 cases were treated with drug-coated balloons and 24 cases were treated with drug-eluting stents(DES).The patients were followed up for 6 months and compare the results of the coronary angiography re-examinations between the two groups.Results There was no statistically significant difference in baseline data between the two groups(P>0.05).The residual stenosis in the drug-eluting stent group was better than the drug-coated balloon group in the two groups of patients immediately after surgery,and the difference was statistically significant(P<0.05).The results of coronary angiography after 6 months of follow-up between the two groups showed no significant difference in target vessel restenosis rate,target vessel revascularization and overall MACE(P>0.05).There were no significant differences statistically in bleeding events between the two groups of patients at 6 months after surgery(P>0.05).Conclusion The drug-coated balloon method is safe and effective for the treatment of patients with coronary artery in-stent restenosis.
