慢性心力衰竭患者血小板指标与左心室射血分数的关系

摘　　要：目的探索慢性心力衰竭患者血小板指标与左心室射血分数(left ventricular ejection fraction,LVEF)的相关性。方法纳入2019年1月至2019年1月渭南市第二医院216例纽约心脏协会(New York Heart Associa⁃tion,NYHA)心功能Ⅱ-Ⅳ级且LVEF≤45%的慢性心力衰竭患者。记录其临床情况、血小板指标、LVEF及其影响因素。Pearson线性相关分析检测LVEF与各影响因素及血小板指标的相关性,存在线性相关的影响因素及血小板指标纳入多元线性回归进行校正分析。结果本研究患者总胆固醇、三酰甘油,高密度脂蛋白胆固醇、血小板计数、血小板压积与LVEF呈显著正相关(P<0.05);血红蛋白、肌酐、总胆红素、糖化血红蛋白与LVEF呈显著负相关(P<0.05);平均血小板体积、血小板分布宽度与LVEF无显著相关性(P>0.05)。使用多元线性回归校正LVEF显著相关的影响因素,如总胆固醇、三酰甘油,高密度脂蛋白胆固醇、血红蛋白、肌酐、总胆红素、糖化血红蛋白后,血小板压积与LVEF显著相关(β=0.648,P=0.008),但血小板计数与LVEF无显著相关性(β=-0.428,P=0.096)。结论在慢性心力衰竭患者中,血小板压积与LVEF呈显著正相关。Objectives To explore the association between various platelet index and left ventricular ejection fraction(LVEF)in patients with chronic heart failure.Methods This study enrolled 216 patients with chronic heart failure,who had a New York Heart Association(NYHA)classification Ⅱ-Ⅳ and LVEF≤45%from January 2019 to December 2019 in Shaanxi Provincial People′s Hospital.The clinical status,platelet index,LVEF and confounding factors were documented.Pearson linear correlation analysis was used to detect the correlation between LVEF and various confounding factors and platelet indicators.The linearly correlated confounding factors and platelet indicators were included in multiple linear regressions for association analysis.Results It was found that total cholesterol,triglyceride,high-density lipoprotein cholesterol,platelet count and plateletocrit significantly and positively correlated with LVEF(P<0.05).Hemoglobin,creatinine,total bilirubin,glycosylated hemoglobin significantly and negatively correlated with LVEF(P<0.05).There was no significant correlation between mean platelet volume,mean platelet distribution width and LVEF(P>0.05).The multiple linear regression adjusted the significant correlated factors with LVEF,namely as total cholesterol,triglycerides,high-density lipoprotein cholesterol,hemoglobin,creatinine,total bilirubin,glycated hemoglobin,plateletocrit significantly associated with LVEF(β=0.648,P=0.008),but platelet count did not signifi⁃cantly associate with LVEF(β=-0.428,P=0.096).Conclusions Plateletocrit significantly and positively associated with LVEF in patients with chronic heart failure.
