摘 要:目的筛选并评价血清循环微小RNA(micro RNA,miR)预测急性心肌梗死(acute myocardial infarction,AMI)后心力衰竭(heart failure,HF)的诊断价值,以期为AMI的诊疗提供新的生物标志物。方法收集2017年1月至2018年12月在成都市第二人民医院40例AMI后HF患者和40例AMI后非HF患者。通过实时定量聚合酶链反应q-PCR筛选候选miR,统计分析差异,计算预测价值。结果筛选出5个候选miRs,经单因素方差分析发现,AMI后HF组的血清循环miR-150水平明显低于AMI后非HF组(0.38±0.25 vs.0.65±0.33,P<0.001)。使用miR-150预测AMI后HF时,受试者工作特征曲线(receiver operating characteristic curve,ROC)下面积(area under the curve,AUC)为0.802(95%CI:0.708-0.895,P=0.000),预测阈值截断值为0.45,对应的灵敏度和特异度分别为67.5%和77.5%。结论血清循环miR-150是预测AMI后HF的新型生物学标志物,但未来仍需进一步扩大样本量进临床研究验证其预测价值。Objectives To explore the prognostic value of the serum circulating micro RNA(miR)-150 on predicting post-acute myocardial infarction(AMI)heart failure(HF),in order to provide a new biomarker for screening AMI.Methods We collected 40 patients with post-AMI HF and 40 patients with post-AMI non-HF from January 2017 to December 2018 in Chengdu Second People′s Hospital.MiR were screened by real-time quantitative polymerase chain reaction(PCR),then statistical analysis was analyzed,and predicted values were calculated.Results Five candi⁃date miRs were screened.One-way ANOVA showed that serum circulating miR-150 levels in post-AMI HF group was significantly lower than that in post-AMI non-HF group(0.38±0.25 vs.0.65±0.33,P<0.001).Further analysis showed that the area under the receiver operating characteristic curve(ROC)was 0.802(95%CI:0.708-0.895,P=0.000).The threshold cut-off value was 0.45,and the corresponding sensitivity and specificity were 67.5%and 77.5%,respec⁃tively when serum circulating miR-150 was used to predict HF after AMI.Conclusions Serum circulating miR-150 is a new biomarker to predict post-AMI HF.Further large sample prospective clinical research is needed to validate its role in the future.
