摘  要:目的 评价能谱CT金属伪影去除(MAR)技术在不同keV条件下减低全膝关节置换物伪影的效果.方法 对38例全膝关节置换术后的患者行能谱CT检查,获取的扫描数据,以每隔10 keV重组出70~140 keV共8组MAR和非MAR图像.比较相同keV条件下MAR组和非MAR组图像分别在股骨髁和胫骨平台下植入物后方肌肉、皮下脂肪和骨髓的CT值、标准差(SD)值和伪影指数(AI),并进行统计学分析.结果 随着keV的递增,MAR组和非MAR组图像在股骨髁和胫骨平台下植入物后方肌肉组织和骨髓的CT值递减、脂肪的CT值递增,所有感兴趣区(ROI)内的SD值和AI呈递减趋势.相同keV条件下,股骨髁和胫骨侧后方肌肉组织、骨髓的CT值和SD值在MAR组均低于非MAR组,而皮下脂肪的CT值相反.所有能级水平的AI在MAR组图像均明显低于非MAR组(P≤0.003).结论 MAR技术可以有效地减少全膝关节置换物伪影,提高解剖结构的清晰度.Objective To evaluate the efet of mtal arifact reduction(MAR)of energy spectrum CT on removing metal artifact of total knee arthroplasty with different keV.Methods 38 patients with total knee arthroplasty were examined by energy spectrum CT.Postoperative CT images were reconstructed with 70-140 keV interval 10 keV.CT value,standard deviation(SD)and arifact index(AI)in the posterior muscle,subcutaneous fat and bone marrow around the implants were compared between MAR and non MAR respectively at the same keV at the femoral condyle and under tibial platform.Then the data were compared with statistics.Results With the increase of keV,the CT value of the posterior muscle and bone marrow around the implants decreased at the femoral condyle and under tibial platform,but the CT value of the subcutaneous fat increased.The SD and Al of all region of interest(ROI)in MAR and non-MAR decreased.With the same keV,the CT value and SD in the posterior muscle and bone marrow were lower in MAR than that in non MAR e the fomoral sendyle and under tibia platform,while the CT value of the euboutaneouo fot were opporiteAt all energy levels,AI was significantly lower in MAR than that in non MAR(P≤0.003).Conclusion MAR can efectively reduce artifact of total knee replacement,improve the clarity of anatomical structure.
