摘　　要：目的探究临床药师参与外科药物治疗管理的工作模式。方法分析某院外科实施ERAS后临床药师工作现状,阐述药师开展加速康复外科围手术期药物治疗管理的工作模式,评估药物治疗管理工作开展成效。结果将MTM服务的理念、方法和技巧逐步运用到加速康复外科临床药学服务中,初步形成临床药师服务模式。结论临床药师逐步优化与ERAS核心要素相关的药物治疗管理路径,对合并基础疾病的患者进行药学评估与监护,使患者得到合理、经济和安全的药物治疗。Objective To explore the working mode of clinical pharmacists participating in the Surgical medication therapy management.Methods Analyze the current status of clinical pharmacist work after the implementation of ERAS in general surgery of our hospital,elaborate the working mode of clinical pharmacists in perioperative medication therapy management of,and evaluate the effectiveness of medication therapy management work.Results The concepts,methods and skills of MTM services were gradually applied to Enhanced recovery after surgery clinical pharmacy service,and a clinical pharmacist service model was formed initially.Conclusion The clinical pharmacist gradually optimizes the management path of drug therapy related to the core elements of ERAS,and conducts pharmaceutical evaluation and monitoring of patients with chronic diseases,so that patients can receive reasonable,economical and safe drug treatment.
