老年前列腺增生尿道等离子电切术后尿路感染与尿道多重耐药菌定植的相关性

摘　　要：目的研究老年前列腺增生尿道等离子电切术后尿路感染与尿道多重耐药菌定植的相关性。方法选取2017年12月—2019年12收治的368例老年前列腺增生尿道等离子电切术患者,根据术后是否发生感染将其分为感染组179例和对照组189例,术后采集所有患者中段尿进行细菌培养,分析尿道多重耐药菌定植率分布情况,并分析影响术后尿路感染的危险因素与多重耐药菌的关系。结果单因素与多因素Logistic分析显示,多重耐药菌定植、术后住院时间、总体治疗费用为影响多重耐药菌与术后感染的危险因素(P<0.05);产β-内酰胺酶细菌、耐西林金黄色葡萄球菌、多重耐药鲍曼不动杆菌、多重耐药铜绿假单胞菌为主要多重耐药菌。结论老年前列腺增生尿道等离子电切术后尿路感染发生风险较高,需对术后尿路感染患者进行病原菌分析,结合患者病情和经济制订合理化治疗方案。Objective To study the correlation between urinary tract infection and colonization of multidrugresistant bacteria in urethra after plasma electrosurgical resection of elderly patients with benign prostate hyperplasia.Methods To selected 368 cases of elderly patients undergoing urethral plasma resection of prostate hyperplasia from December 2017 to December 2019,and they were divided them into an infection group with 179 cases and a control group with 189 cases according to whether infection occurred after the operation.The mid-section urine of all patients was collected after the operation.Bacterial culture was performed to analyze the distribution of the colonization rate of multi-drug resistant bacteria in the urinary tract,and analyze the relationship between the risk factors of postoperative urinary tract infection and multi-drug resistant bacteria.Results Univariate and multivariate Logistic analysis showed that the multi-drug-resistant bacteria colonization,postoperative hospital stay,and overall treatment costs were the risk factors that affected multi-drug-resistant bacteria and postoperative infection(P<0.05).Theβ-lactamase producing bacteria,cillin-resistant Staphylococcus aureus,multi-drug resistant Acinetobacter baumannii,and multi-drug resistant Pseudomonas aeruginosa were the main multi-drug resistant bacteria.Conclusion The risk of urinary tract infection after plasma electrosurgical resection of prostate hyperplasia in elderly patients was relatively high.It was necessary to analyze the pathogenic bacteria of patients with postoperative urinary tract infection,and to formulate a rational treatment plan based on the patient's condition and economy.
