摘  要:目的:探讨两种不同呼吸模式在COPD呼吸衰竭机械通气患者中的治疗效果方法:选取2019年1月-2019年12月来我院就治的130例COPD呼吸衰竭患者,随机分为两组,A组采取自主呼吸+双水平正压通气模式65例,B组采取同步间歇指令通气+压力支持通气模式65例,观察两组治疗前后的动脉血PaO2、PaCO2、PH、呼吸频率、心率改变,比较治疗效果。结果:治疗后,两组患者的动脉血PaO2、PH有显著升高,PaCO2、呼吸频率和心率有显著下降-两组间比较B组较A组PaCO2、呼吸频率和心率明显降低,动脉血PaO2、PH有显著升高结论:此两种呼吸模式在治疗COPD呼吸衰竭机械通气患者的效果显示,两种呼吸方式都有明显疗效,并且同步间歇指令通气+压力支持通气模式疗效更为显著,值得在临床推广应用.Objective:To explore the therapeutic effect of two different respiratory modes in patients with COPD respiratory failure mechanical ventilation.Methods:130 cases of COPD patients with respiratory failure treated in our hospital from January 2019 to December 2019 were randomly divided into two groups,65 cases in group A were treated with spontaneous breathing+bi level positive pressure ventilation,and 65 cases in group B were treated with synchronous intermittent mandatory ventilation and pressure support ventilation After the arterial blood PaO2,PaCO2,pH,respiratory rate,heart rate changes,compare the treatment effect.Results:after treatment,PaO2 and pH in arterial blood were significantly increased,PaCO2,respiratory rate and heart rate were significantly decreased in both groups-compared with group A,PaCO2,respiratory rate and heart rate in group B were significantly lower than those in group A,PaO2 and pH in arterial blood were significantly increased.Conclusion:the effect of these two respiratory modes in the treatment of COPD respiratory failure patients with mechanical ventilation shows that both breathing modes have obvious curative effect And the effect of synchronized intermittent mandatory ventilation+pressure support ventilation mode is more significant,which is worthy of clinical application。
