摘　　要：目的比较低温等离子靶点消融术与射频靶点热凝术联合臭氧治疗神经根型颈椎间盘突出症(RCDH)的疗效和对血清IL-6、IL-8水平的影响。方法选择148例RCDH患者,随机分成2组,每组各74例。观察组采用低温等离子靶点消融术联合臭氧治疗;对照组采用射频靶点热凝术联合臭氧治疗。分别于术前及术后不同时间点评估患者的VAS得分和颈椎功能障碍指数(NDI),检测患者血清IL-6、IL-8活性,并评价其疗效。结果两组患者的VAS评分和NDI,术后均比术前显著减少(P<0.01),观察组在术后4天时高于对照组,但在术后3及6个月时均较对照组显著减少(P<0.05和0.01)。两组患者术后IL-6、IL-8水平均较术前明显下降(P<0.01),术后3月时观察组下降水平明显低于对照组(P<0.05和0.01)。观察组疗效优良率明显高于对照组(P<0.05)。结论低温等离子靶点消融术联合臭氧比射频靶点热凝术联合臭氧的疗法能更好地改善RCDH患者血清炎症因子水平,取得更好的疗效。Objective To compare the efficacy of low-temperature plasma target ablation(LPTA)and radiofrequency target thermocoagulation(RTT)combined with ozone injection in the treatment of radiculectomy cervical disc herniation(RCDH)and the effects on serum IL-6 and IL-8 levels.Methods 148 patients with RCDH were randomly divided into two groups,with74 cases in each group.The observation group was treated with LPTA combined with ozone injection;the control group was treated with RTT combined with ozone.At different time points before and after operation,the patients’VAS score and the neck disability index(NDI)were assessed respectively and their serum IL-6 and IL-8 activities were detected,and their efficacy was evaluated.Results The VAS score and NDI in the two groups were significantly reduced after surgery(P<0.01).The observation group was higher than the control group at 4 th days after surgery,but was significantly lower at the 3 rd and 6 th month after surgery(P<0.05 and 0.01).The levels of IL-6 and IL-8 in the two groups were significantly lower after operation(P<0.01),and the observation group was significantly lower than the control group at the 3 rd month after operation(P<0.05 and 0.01).The excellent and good rate of curative effect in the observation group was significantly higher than that in the control group(P<0.05).Conclusion LPTA combined with ozone can better improve the level of inflammatory factors in RCDH patients’serum and achieve better curative effects than RTT combined with ozone therapy.
