摘 要:目的探讨糖尿病足溃疡创面感染患者创面渗液基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-2抑制因子(TIMP-2)、Smad3蛋白检测的临床意义。方法选取广州市花都区人民医院2017年6月至2019年12月收治的60例糖尿病足溃疡创面感染患者作为研究组,另选取同期前来我院就诊的60例糖尿病足溃疡未伴创面感染患者作为对照组。两组均接受创面渗液MMP-2、TIMP-2、Smad3蛋白检测,记录并比较两组间、研究组内患者(单一致病菌感染者及混合致病菌感染者、治疗前及感染控制后)上述指标的检测结果。结果研究组MMP-2、Smad3蛋白水平显著高于对照组,研究组TIMP-2水平低于对照组(P<0.05)。研究组中单一致病菌感染者MMP-2、Smad3蛋白水平显著低于混合致病菌感染者,单一致病菌感染者TIMP-2水平高于混合致病菌感染者(P<0.05)。研究组经对症治疗感染控制后MMP-2、Smad3蛋白水平较治疗前显著下降,TIMP-2水平则有所上升(P<0.05)。结论糖尿病足溃疡患者检测溃疡创面MMP-2、TIMP-2、Smad3蛋白对溃疡创面感染的有一定的诊断价值。Objective To investigate the clinical significance of detection of matrix metalloproteinase 2(MMP-2), matrix metalloproteinase 2 inhibitory factor(TIMP-2) and Smad3 protein in diabetic foot ulcer wound infection. Methods Sixty patients with diabetic foot ulcer wound infections admitted to the People’s Hospital of Huadu District from June 2017 to December 2019 were selected as the research group, and sixty patients with diabetic foot ulcers without wound infections who came to the People’s Hospital of Huadu District during the same period were selected as control group. The levels of MMP-2, TIMP-2 and Smad3 proteins in wound exudate were detected in both groups. The results of these indicators were recorded and compared between the two groups and among patients in the study group(single pathogen infection and mixed pathogen infection, before treatment and after infection control) s. Results The protein levels of MMP-2 and Smad3 in the study group were significantly higher than those in the control group, and the levels of TIMP-2 in the study group were lower than those in the control group(P<0.05). In the study group, the levels of MMP-2 and Smad3 proteins in mono-pathogen infected patients were significantly higher than those in mixed pathogen infected patients, and the TIMP-2 levels in mono-pathogen infected patients were lower than those in mixed pathogen infected patients(P<0.05). The levels of MMP-2 and Smad3 proteins in the study group were significantly decreased after symptomatic treatment and infection control, while the levels of TIMP-2 were increased(P<0.05). Conclusions The detection of MMP-2, TIMP-2, Smad3 protein has positive clinical value on diagnostic value of wound infection in patients with diabetic foot ulcer.
