摘 要:目的探讨颅脑外伤患者有创颅内压监测术后早期颅内感染危险因素。方法纳入2016年1月至2019年10月本院重症医学科住院治疗的颅脑外伤患者有创颅内压监测患者120例,根据是否出现术后早期颅内感染分为感染组(37例)和非感染组(83例),比较两组临床资料和检验资料的差异,采用多因素Logistic回归分析相关危险因素。结果感染组年龄≥50岁、接受后颅窝手术、手术时间≥4小时、糖尿病患者占比高于非感染组(P<0.05),感染组GCS评分低于非感染组,探头留置时间高于非感染组(P<0.05)。多因素Logistic回归显示,年龄、后颅窝手术、GCS评分、探头留置时间是颅脑外伤患者有创颅内压监测术后早期颅内感染的独立危险因素。结论颅脑外伤患者有创颅内压监测术后早期颅内感染具有较多危险因素,年龄增加、后颅窝手术、低GCS评分和探头留置时间长均会增加颅内感染风险。Objective To investigate the risk factors of early intracranial infection in patients with traumatic brain injury after invasive intracranial pressure monitoring. Methods A total of 120 patients with traumatic intracranial pressure monitoring in patients with craniocerebral trauma who were hospitalized in intensive care unit from January 2016 to October 2019 were included. They were divided into infection groups(37 cases) and non-infected group(83 cases). The clinical data and test data of the two groups were compared, and related risk factors were analyzed by multivariate logistic regression. Results The proportion of patients with the age ≥50 years, the posterior cranial fossa surgery, the operation time≥4 h, and diabetes was higher than that of the non-infected group(P<0.05). The GCS score was lower than those in the non-infected group, and probe indwelling time in the infected group were higher than those in the non-infected group( P <0.05). Multivariate logistic regression showed that age,posterior cranial fossa surgery, GCS, and probe indwelling time were independent risk factors for early intracranial infection after invasive intracranial pressure monitoring in patients with traumatic brain injury. Conclusion Patients with traumatic brain injury have more risk factors for early intracranial infection after invasive intracranial pressure monitoring. Increasing age, posterior fossa surgery, low GCS score, and probe indwelling all increase the risk of infection.
