摘 要:目的探讨急性缺血性脑卒中(AIS)患者粪便中巨单胞菌属和大肠杆菌属对AIS的诊断效能。方法选取2015年5月-2017年10月天津市环湖医院收治的AIS患者10例为AIS组,选取同期体检健康者10例为健康组,通过16S测序技术分析AIS患者粪便中巨单胞菌属和大肠杆菌属相对丰度,通过ROC分析考察巨单胞菌属和大肠杆菌属对AIS的AUC面积、灵敏度、特异度等诊断效能以及cut-off值。结果同健康组比较,AIS组患者肠道中巨单胞菌属相对丰度上调,大肠杆菌属相对丰度下调,差异均有统计学意义(P<0.05)。通过ROC分析发现巨单胞菌属、大肠杆菌属以及二者比值的AUC面积、灵敏度、特异度和cut-off值分别为0.612 5、0.762 5、0.625 0,63%、63%、88%,60%、70%、60%,0.0043、0.072 0、0.144 1。结论 AIS患者粪便中巨单胞菌属和大肠杆菌属变化可能与AIS疾病进程相关,可作为AIS的潜在诊断指标。Objective To explore the diagnostic efficiency of Megamonas and Escherichia in stool of patients with acute ischemic stroke(AIS).Methods Ten patients with AIS admitted to Tianjin Huanhu Hospital from May 2015 to October 2017 were selected as the AIS group,and 10 healthy people were selected as the control group.The relative abundance Megamonas and Escherichia in the stool of patients with AIS were analyzed by 16 S sequencing technology,and the diagnostic efficiency of Megamonas and Escherichia of AIS on AUC area,sensitivity,specificity and cut-off value were investigated by ROC analysis.Results Compared with the healthy group,the relative abundance of Megamonas was upregulated and the relative abundance of Escherichia was down-regulated in AIS group,and the differences were statistically significant(P<0.05).ROC analysis showed that the AUC area,sensitivity,specificity and cut-off value of Megamonas,Escherichia and Megamonas/Escherichia were 0.612 5,0.762 5,0.625 0,and 63%,63%,88%,and 60%,70%,60%,and 0.004 3,0.072 0,0.144 1.Conclusion The changes of intestinal Megamonas and Escherichia in the stool of patients with AIS might be related to the progression of AIS,which could be used as a potential diagnostic indicator of AIS.
