摘　　要：目的评估应用缝合锚钉结合钩钢板固定治疗Neer Ⅱ B型锁骨远端骨折的疗效。方法回顾性分析2014年1月至2018年12月秦皇岛市第一医院采用缝合锚钉重建喙锁韧带结合钩钢板固定治疗21例Neer Ⅱ B型锁骨远端骨折患者资料。所有患者均为新鲜闭合骨折,且未合并血管、神经损伤。其中,男14例,女7例,年龄21~63岁,平均38岁。结果术后切口均Ⅰ期愈合。所有患者均获随访,随访时间12~20个月,平均17个月。最终X线片示骨折均愈合,愈合时间6~20周,平均9周。术后内固定物未出现失效情况,且内固定取出后无再发肩锁关节脱位。结论在严格掌握手术适应证及手术技巧的前提下,缝合锚钉结合钩钢板固定治疗不稳定型NeerⅡ B型锁骨远端骨折效果显著,具有恢复快及并发症少的特点,并且可获得接近解剖的喙锁间距及良好的肩关节功能。Objective To evaluate the efficacy of suture anchor combined with hook plate fixation in the treatment of NeerⅡtype B distal clavicle fracture.Methods From January 2014 to December 2018,a retrospective study was done to analyze 21 cases of NeerⅡtype B distal clavicle fractures who were treated with suture anchor reconstruction of coracoclavicular ligament combined with hook plate fixation in Qinhuangdao First Hospital.All patients were fresh closed fractures without vascular and nerve injury.There were 14 males and 7 females with an average age of 38 years(range,21-63 years).Results All the incisions healed in the first stage.All patients were followed up for an average of 17 months(range,12-20 months).X-ray showed that all fractures healed.The healing time was 6-20 weeks(average 9 weeks).There was no failure of internal fixation and no acromioclavicular joint dislocation occurred after removal of internal fixation.Conclusion Under the premise of strictly mastering the surgical indications and surgical skills,suture anchor combined with hook plate fixation is effective in the treatment of unstable NeerⅡtype B distal clavicle fracture.It has the characteristics of quick recovery and fewer complications,and can obtain close to anatomical coracoid lock distance and good shoulder joint function.
