隐球菌性脑膜炎的临床表现及墨汁染色联合隐球菌抗原检测的诊断意义

摘　　要：目的探讨隐球菌性脑膜炎的临床表现及墨汁染色联合隐球菌抗原检测的诊断意义。方法选取2018年1月至2019年6月本院收治的中枢神经系统感染患者40例。其中10例隐球菌性脑膜炎患者作为观察组,其他30例中枢神经系统感染患者作为对照组,比较两组患者临床症状表现。采用墨汁染色法和墨汁染色联合隐球菌抗原检测(联合检测)患者脑脊液中隐球菌阳性率,比较两种方法的阳性检测结果及敏感度。结果两组患者头痛、呕吐、发热、视力下降、盗汗、听力下降及意识水平下降等临床症状表现比较差异无统计学意义;观察组中联合检测阳性率为90.00%,高于墨汁染色的50.00%(P<0.05);联合检测敏感度为90.00%,高于墨汁染色的50.00%(P<0.05),两种检测方法特异度、阳性预测值及阴性预测值比较差异均无统计学意义。结论墨汁染色联合隐球菌抗原检测隐球菌性脑膜炎的阳性率及敏感度高,临床诊断价值较高。Objective To investigate the clinical manifestations of cryptococcal meningitis and the diagnostic significance of Chinese ink staining combined with detection of cryptococcal antigen.Methods 40 inpatients with central nervous system infection were collected from our hospital from January 2018 to June 2019.10 cases of cryptococcal meningitis were selected as the observation group and 30 cases of other central nervous system infections were selected as the control group.The clinical symptoms of the two groups were analyzed.The positive rate of cryptococcus in cerebrospinal fluid was detected by ink staining,ink staining and cryptococcus antigen detection(combined detection).The positive results and sensitivity of the two methods were compared.Results There was no significant difference in headache,vomiting,fever,vision loss,night sweats,hearing loss and consciousness level between the two groups.In the observation group,the positive rate of combined detection was 90.00%,which was higher than that of ink staining(50.00%,P<0.05).The sensitivity of combined detection was 90.00%,which was higher than that of ink staining(50.00%,P<0.05).The specificity,positive predictive value and negative predictive value of the two detection methods showed no statistical significance.Conclusion Ink staining combined with cryptococcal antigen detection of cryptococcal meningitis has high positive rate and sensitivity,and has high clinical diagnostic value.
