DHS与InterTan内固定治疗老年股骨粗隆间骨折的疗效对比及对VAS评分的影响

摘　　要：目的探讨老年股骨粗隆间骨折患者应用DHS与InterTan内固定治疗的疗效及对VAS评分的影响。方法选取2014年1月至2016年12月本院骨外科收治的90例老年股骨粗隆间骨折患者,随机分为两组,各45例。对照组患者接受DHS内固定治疗,研究组患者接受InterTan内固定治疗,比较两组患者临床疗效、手术情况、术后康复情况、并发症及术后3个月髋关节功能和疼痛程度。结果两组患者骨折愈合时间比较差异无统计学意义。研究组手术时间、术中出血量、住院时间、术后下地时间以及并发症发生率均显著低于对照组(P<0.05)。术后3个月,研究组患者Harris评分(85.52±7.47)分显著高于对照组(75.54±5.52)分,VAS评分(3.35±1.16)分显著低于对照组(5.32±1.53)分,临床疗效优良率(91.11%)明显高于对照组(75.56%),差异有统计学意义(P<0.05)。结论与DHS内固定比较,在老年股骨粗隆间骨折患者治疗中应用InterTan内固定治疗,能明显缩短手术时间,且手术创伤小,术后并发症少,促进患者术后髋关节功能的恢复,预后效果理想,值得临床推广应用。Objective To investigate the effect of DHS and InterTan internal fixation in elderly patients with intertrochanteric fracture and its effect on VAS score. Methods A total of 90 elderly patients with intertrochanteric fracture treated in our hospital from January 2014 to December 2016 were randomly divided into two groups, with 45 cases in each group. The control group received DHS internal fixation, and the study group received InterTan internal fixation. Clinical efficacy, surgical condition, postoperative rehabilitation, complications, hip function and pain degree three months after operation were compared between the two groups. Results There was no significant difference in the healing time between the two groups. The duration of operation, intraoperative blood loss, length of hospital stay, postoperative time to the ground and the incidence of complications in the study group were significantly lower than those in the control group(P<0.05). The Harris score(85.52±7.47) scores and the VAS score(3.35±1.16) scores in the DHS group at three months after operation were significantly lower than those in the control group(5.32±1.53) scores and(5.32±1.16) scores. The excellent clinical efficacy rate(91.11%) was significantly higher than that of the control group(75.56%), the difference was statistically significant(P<0.05). Conclusion Compared with DHS internal fixation, InterTan internal fixation can be used to treat elderly patients with intertrochanteric fractures, which can shorten the operation time, reduce trauma and avoid postoperative complications, and promote the function of hip joint after operation recovery, the prognosis is ideal, it is worth clinical application.
