摘  要:目的通过应用表皮生长因子(epidermal growth factor,EGF)与安慰剂(或空白组)治疗鼓膜穿孔的疗效对比,系统评价EGF治疗创伤性鼓膜穿孔的临床疗效。方法检索PubMed、EMBASE、Cochrane、万方和知网等数据库中使用EGF治疗鼓膜穿孔的临床随机对照实验,搜索时间截止于2020年1月,运用RevMan5.3和STATA12软件进行Meta分析。结果本项研究共纳入8个随机对照实验研究,共631例创伤性鼓膜穿孔患者。Meta分析显示,与安慰剂(或空白组)比较,EGF能显著提高鼓膜穿孔愈合率[RR=1.27,95%CI(1.17,1.37),P<0.05,I^(2)=37%],明显缩短穿孔愈合时间[SMD=-1.70,95%CI(-1.97,-1.43),P<0.05,I 2=0%],听力功能改善方面较对照组比较,差异无统计学意义[SMD=0.22,95%CI(-0.1,0.53),P=0.18,I 2=0%]。结论应用EGF治疗创伤性鼓膜穿孔是一种有效、方便且安全的治疗方式。Objective To investigate the effectiveness of epidermal growth factor(EGF)versus placebo or no intervention for the repair of traumatic tympanic membrane(TM)perforations in the controlled clinical trials.Methods Databases,including PubMed,EMBASE,Cochrane,Wanfang and CNKI were screened from their inceptions to January 2020.Subsequently,meta-analysis was performed to calculate the data using the RevMan(version 5.3)and STATA(version 12)software.Results The meta-analysis results have revealed that the application of EGF can significantly enhance closure rates of the TM perforations[RR=1.27,95%CI(1.17,1.37),P<0.05,I^(2)=37%]and shorten healing time during TM regeneration[SMD=-1.70,95%CI(-1.97,-1.43),P<0.05,I 2=0%].In terms of hearing improvement,the current evidence is still limited[SMD=0.22,95%CI(-0.1,0.53),P=0.18,I 2=0%].Conclusions EGF is effective for the patients with traumatic TM perforations owing to its ease of handling,availability,and safety.
