上前牙即刻种植后即刻修复与使用愈合基台效果的对比研究

摘　　要：目的比较使用临时义齿即刻修复和直接使用愈合基台对上前牙即刻种植疗效的影响。方法选择适合即刻种植的上前牙病例,种植体植入后分组行临时义齿即刻修复和愈合基台封闭牙龈,术后6个月开始牙龈塑形,最后行氧化锆全瓷冠永久修复。通过测量不同时期种植体颈缘骨吸收量、牙龈红色美学评分(PES)来比较两组间的差异。结果两组种植体均能完成良好的骨结合、行使功能以及形成软组织美学。完成修复后不同时期,种植体颈缘骨吸收量即刻修复组少于愈合基台组,尤其在完成修复时有统计学意义(P<0.05),两组在完成修复后6个月、12个月时的骨吸收量比较稳定;牙龈红色美学评分即刻修复组高于愈合基台组,且在完成修复时有统计学意义(P<0.05),但是即刻修复组在完成修复后6个月时美学评分降低(P<0.05),愈合基台组牙龈红色美学评分一直比较稳定。结论即刻种植后两种牙龈封闭方式对维持牙槽骨的高度和形态以及恢复前牙区美观效果都有重要的作用,即刻修复组短期内疗效更好,两组远期效果无明显差别,顶部膨大的愈合基台在即刻种植后对牙龈美学方面的有其独特的适应症和作用。Objective To compare the effects of using immediate restoration and healing abutment after immediate implantation in anterior maxillary teeth. Methods Choosing appropriate patients needing immediate implantation in anterior maxillary teeth,and respectively using immediate restoration and healing abutment to close the gingival defects,and restoring the teeth with permanently zirconia porcelain crowns finally. Then compare the differences between the two groups by measuring the bone resorptions around the implant neck and the gingival pink esthetic score(PES). Results Implants osseointegration were maintained,with good functions and higher soft tissue aesthetics at the final restoring time,then after 6 months,and 12 months,respectively. It showed that the bone resorptions was lower in the immediate restoration group than in the healing abutment group, especially at the final restoring time(P <0.05),and the bone resorptions were stable at the 6 months time and 12 months time. The PES was higher in the immediate restoration group than in the healing abutment group, and it was statistical significance at the final restoring time(P<0.05),but the PES reduced within the immediate restoration group at the 6 and 12 months time,while that was always stable in the healing abutment group. Conclusion The two kinds of approach both have important role in maintaining the height and shape of alveolar bone and in recovering the aesthetic effect in anterior tooth area. It was better with the immediate restoration group in short term,and the differences of the two groups were less in late time,and it has its distinctive indications and roles with the healing abutment.
