摘　　要：目的评价人文关怀在ICU气管切开患者护理中的应用效果。方法检索The Cochrane Library、Pubmed、Embase、迈特思创医学知识库、中国生物医学文献服务系统(Sinomed数据库)、中国维普全文数据库(VIP)、中国知网全文数据库(CNKI)、万方数据库中有关ICU气管切开患者进行人文关怀护理的随机对照试验,检索时间为建库至2019年9月。同时检索相关文献中的原始文献和综述中所附参考文献,2名研究者独立进行文献检索、文献筛选、资料提取、纳入文献方法学质量的评价并交叉核对,应用RevMan 5.5软件进行Meta分析。结果共纳入10篇随机对照试验进行Meta分析,包括917例研究对象,干预组459例,对照组458例。Meta分析结果显示,与常规护理相比,人文关怀护理有效缓解ICU气管切开患者的焦虑情绪[MD=-6.76,95%CI(-10.74,-2.78),P<0.001],提高护理满意度[RR=1.26,95%CI(1.18,1.34),P<0.001]、[MD=10.72,95%CI(5.93,15.51),P<0.001],降低气管切开术后并发症的发生率[RR=0.17,95%CI(0.09,0.32),P<0.001]。结论人文关怀护理对ICU病房气管切开患者有效缓解焦虑情绪、提高护理满意度、降低气管切开术后并发症的发生率有积极影响。Objective To evaluate the application effect of humanistic care nursing in ICU patients with tracheotomy.Methods Randomized controlled trials on the effectiveness of humanistic nursing for ICU patients with tracheotomy before September,2019 were retrieved from electronic database of Cochrane Library,PubMed,Embase,Metstr medical knowledge base,Service system of Chinese biomedical literature database(Sinomed),VIP,CNKI,and Wan Fang data,and the original literatures and references attached to the reviews were also searched.Two researchers independently screened potentially eligible articles,extracted data and assessed risk of bias.RevMan 5.5 software was used to perform Meta-analysis.Results A total of 10 randomized clinical trials were included in the Mate analysis,including 917 subjects,459 in the intervention groups and 458 in the control groups.The results of Meta-analysis showed that,compared with routine nursing,humanistic care nursing can effectively relieve anxiety of ICU patients with tracheotomy[MD=-6.76,95%CI(-10.74,-2.78),P<0.001],and enhance nursing satisfaction[RR=1.26,95%CI(1.18,1.34),P<0.001],[MD=10.72,95%CI(5.93,15.51),P<0.001],and reduce the incidence of complications after tracheotomy[RR=0.17,95%CI(0.09,0.32),P<0.001].Conclusions Humanistic care nursing can effectively relieve anxiety,and improve nursing satisfaction and reduce the incidence of complications after tracheotomy in ICU patients with tracheotomy.
