摘　　要：目的报道4例来自海南省的掌黑癣患者临床资料,为掌黑癣的诊断及治疗提供科学依据。方法回顾性总结海南省第五人民医院皮肤科2016—2018年确诊的4例掌黑癣患者的临床特点。结果4例患者均为海南省海口市人。其中女性3例,男性1例;4岁以下3例,成人1例;3例患儿均喜好玩沙,1例成人有手足多汗症病史。4例患者皮损部位均为手掌,原发皮疹为黑色斑疹,患者无自觉症状。根据临床表现、10%KOH涂片直接镜检、真菌培养和分子生物学鉴定确诊为掌黑癣,分离菌株鉴定为威尼克何德霉。4例患者通过外用抗真菌制剂治疗后治愈,疗程5~14 d。结论掌黑癣在海南省发病率低可能被低估。海南省报道的掌黑癣病例主要是儿童。通过真菌直接镜检可快速诊断,真菌培养和分子生物学鉴定可明确致病菌种。外用抗真菌制剂2周内可完全治愈本病。Objective We reported the clinical data of four cases with tinea nigra from Hainan Province,to provide scientific basis for the diagnosis and treatment of tinea nigra.Methods The clinical characteristics of four patients with tinea palmaris who visited dermatology clinic at the Fifth People’s Hospital of Hainan Province from 2016 to 2018 were analyzed and summarized retrospectively.Results Four cases of tinea nigra were from Haikou City,Hainan Province.Among them,there were three female,and one was male.Three patients were under 4 years old,and one patient was adult.Three kids loved playing with the sand,and the adult has a history of palmar and foot hyperhidrosis.The skin lesions of the 4 patients were all on the palm,and the primary skin rash was black macular.The patients had no conscious symptoms.Diagnosis was confirmed according to clinical manifestation,10%KOH microscopic examination,fungal culture and molecule biology method.The causative fungus in our cases was determined to be Hortaea werneckii.All patients cured with topical treatment of antifungal agents for 5 to 14 days.Conclusions We propose that tinea nigra may be under-reported in Hainan Province.Tinea nigra in Hainan Province is more prevalent among children.Direct microscopic examination is useful for quick diagnosis.Moreover,confirmation of identification depends on fungal culture and(or)DNA sequencing.The lesions cleared after treatment with topical antifungal agents within 2 weeks.
